요충(Enterobius vermicularis)기생충은 주로 대장관 직장관 속과 항문 내에서 기생한다.
● 요충으로 생긴 증상 징후를 요충증이라고 한다.
● 요충의 길이는 0.8~1.3cm이고 실같이 가늘고 둥글고 하얗게 생긴 기생충이다.
● 요충은 세계 어느 나라에서도 발견되고 빈부 차이 없이 누구에게나 기생 할 수 있다.
● 영유아들, 학령기 아이들을 비롯해서 사춘기 아이들, 성인들에게도 기생한다.
■ 요충증의 증상 징후
● 요충이 위장관이나 내외부생식기 등 신체 거의 어느 부위에나 기생한다.
● 요충의 수가 얼마나 많이 있느냐 등에 따라 증상 징후가 다르다.
● 요충은 대장관 속, 직장관 속과 항문 속에서 주로 기생하지만 요충의 수가 상당히 많아지면 그 쟝관 속에 있는 요충이 항문, 항문 주위에 있는 피부와 외부 생식기까지 나와서 기어 다닌다.
● 암컷이 항문 밖으로 나와 항문 주위에 있는 피부에 알을 낳기도 한다.
● 요충이 항문이나 그 주위의 피부, 외부생식기와 그 주위에서 기어 다닐 때는 근질근질 가렵고 그로 인해 긁을 수 있다.
● 특히 밤에 요충이 항문의 주위 피부로 기어 다닐 때는 더 많이 가려워 잠을 잘 수 없을 때도 있다.
● 잠자던 아이가 요충 때문에 이를 “갈고 오줌을 싼다.”라는 말도 있지만 사실상 근거가 없는 말이다.
● 드물게 요충 감염으로 질염이나 난관염도 생길 수 있고 간혹 복막염, 골반내염, 또는 충수염도 생길 수 있다.
■ 요충증의 진단
● 병력, 증상 징후, 진찰소견을 종합해서 요충증을 비교적 쉽게 진단할 수 있다.
● 요충은 직장관 속, 항문 속에서 기생하다가 항문과 그 바깥 주위 피부에 나와서 기어 다니면서 알을 깔 수 있다.
● 아침에 일어나자마자 항문 주위의 피부에 끈적거리고 투명한 스카치테이프를 붙였다가 바로 뗀 후, 그 테이프에 어미 요충과 요충 알이 붙어 있나 현미경으로 검사해 진단할 수 있다.
● 그 테이프에 묻은 어미 요충이나 요충 알을 보고 요충증을 진단할 수 있다.

보통 어미 요충은 한 밤중에 항문이나 그 주위의 피부에서 알을 깐다.
● 아침에 일어나자마자 목욕이나 샤워를 하기 전에 스카치테이프를 항문 주위 피부에 붙였다가 바로 떼어야 한다.
● 어린아이가 항문이나 외음부가 가렵다고 호소할 때는 항문과 외음부에 전등불을 비춰 자세히 살펴보면 어미 요충들이 기어 다니는 것도 육안으로 볼 수 있다.
● 또 아침에 일어나자마자 글리세린을 묻힌 면봉 끝으로 항문 주위의 피부를 살살 닦아주는 것처럼 훔친다.
● 그리고 그 면봉의 끝 부위를 슬라이드에 묻힌다. 그 슬라이드를 현미경으로 들여다보면 요충의 알이 있는지 없는지 검사해서 요충증을 진단할 수 있다.
■ 요충증의 치료
Emverm (mebendazol)으로 치료 하면 95% 치료 효과가 있다고 한다. AAP NEWS 9/2018
● 피란텔(Pyrantel), 피페라진(Piperazine), 또는 메벤다졸(Mebendazole)을 1~2회 경구로 섭취해서 치료한다.
● 1차 복용 후 2주 간격을 두고 1회 더 복용해 2차 치료를 하면 대개 요충증은 완전히 치료될 수 있다.
● 요충증이 있는 사람이 사용한 변기에 요충의 성충과 요충의 알이 묻어 있을 수 있다.
● 다른 사람이 그 변기를 쓸 때나 항문 부위나 외음부의 피부를 손으로 긁거나 만진 후 그 손을 깨끗이 씻지 않고 그 손으로 음식물을 먹을 때 요충의 알이 자신의 입을 통과해 위장관 속으로 들어가서 자가 감염이 될 수 있다.
● 손을 깨끗이 씻는 습관이 이때도 중요하다.
● 직장관 속에 있는 대변을 관장으로 배변시키면 어미 요충이 대변과 같이 나올 수 있다.
● 이런 식으로 치료하면 직관장 속의 요충의 수는 일시적으로 다소 감소될 수 있지만 요충증을 근본적으로 치료될 수 없다.
● 메벤다졸은 부작용이 거의 없고 값도 싸며 약 먹기도 좋고 치료효과도 좋다.
● 관장으로 치료하거나 항문에 어떤 약을 발라서 치료할 필요도 없다.
● 의사의 처방에 따라 메벤다졸 등 요충치료약을 복용해서 치료하는 것이 좋다.

■ 요충증의 예방
● 집안 식구들 중 누군가가 요충증을 가지고 있을 때는 다른 식구들도 요충증을 가지고 있을 가능성이 많다.
● 요충증을 가지고 있는 소아청소년 자녀만 치료하지 말고 집안 온 식구들이 동시 치료받는 것이 좋다.
● 그러나 요충증의 증상을 가지고 있거나 요충증이 있다고 확진을 받은 식구만 치료하는 것이 좋다고 하는 의사들도 있다.
● 요충 알이 한 집안 온 식구들에게 감염될 수도 있지만 평소에 같이 노는 친구들이나 탁아소에서 같이 생활하는 다른 아이들에게도 감염될 수 있다.
● 음식물을 먹기 전에 손을 깨끗이 씻는 습관으로 요충증을 예방하고 요충 알이 변기에 묻어 다른 사람에게도 감염될 수 있으므로 변기를 깨끗이 씻어 요충증을 예방해야 한다.

Adelberg Pinworm (Enterobius vermicularis) parasites mainly live in the colon, rectum, and anus. ● Symptoms caused by pinworms are called pinworm disease. ● Pinworms are 0.8 to 1.3 cm long and are thin, round, white parasites like thread. ● Pinworms are found in any country in the world and can parasitize anyone regardless of rich or poor. ● It lives in infants, school-age children, adolescent children, and adults. ■ Symptoms and signs of pinworm disease ● Pinworms live in almost any part of the body, including the gastrointestinal tract and internal and external genitalia. ● Symptoms and signs vary depending on how many pinworms there are. ● Pinworms mainly live in the large intestine, rectum, and anus, but when the number of pinworms increases significantly, the pinworms in the intestinal tract come out and crawl on the anus, the skin around the anus, and the external genitalia. ● The female may come out of the anus and lay eggs on the skin around the anus. ● When pinworms crawl around the anus, the skin around it, or the external genitalia, it becomes itchy and can cause scratching. ● Especially at night, when pinworms crawl on the skin around the anus, it becomes more itchy and you may not be able to sleep. ● There is a saying that a sleeping child “grinds his teeth and urinates” due to pinworms, but this has no basis in fact. ● In rare cases, pinworm infection may cause vaginitis or salpingitis, and occasionally peritonitis, pelvic inflammatory disease, or appendicitis may also occur. ■ Diagnosis of pinworm disease ● Pinworm disease can be diagnosed relatively easily by combining history, symptoms, signs, and examination findings. ● Pinworms live in the rectal tube and anus, then emerge and crawl around the anus and the surrounding skin to lay eggs. ● As soon as you wake up in the morning, you can make a diagnosis by attaching sticky, transparent scotch tape to the skin around the anus, immediately removing it, and inspecting the tape under a microscope to see if mother pinworms and pinworm eggs are attached to the tape. ● Pinworm disease can be diagnosed by looking at the mother pinworm or pinworm eggs on the tape.

Copyright ⓒ 2011 John Sangwon Lee, MD, FAAP Usually, the mother pinworm lays eggs in the anus or the skin around it in the middle of the night. ● As soon as you wake up in the morning, before taking a bath or shower, apply scotch tape to the skin around the anus and immediately remove it. ● When a young child complains that the anus or vulva is itchy, if you shine a light on the anus and vulva and look closely, you can see the mother pinworms crawling around with the naked eye. ● As soon as you wake up in the morning, gently wipe the skin around the anus with the tip of a cotton swab dipped in glycerin. ● Then, place the tip of the cotton swab on the slide. By looking at the slide under a microscope, you can diagnose pinworm disease by examining the presence or absence of pinworm eggs. ■ Treatment of pinworm disease Treatment with Emverm (mebendazol) is said to be 95% effective. AAP NEWS 9/2018 ● Treat by taking Pyrantel, Piperazine, or Mebendazole orally once or twice. ● In most cases, pinworm disease can be completely cured by performing secondary treatment by taking one more dose at two-week intervals after the first dose. ● Toilets used by people with pinworm disease may contain adult pinworms and eggs. ● When another person uses the toilet or scratches or touches the skin of the anus or vulva and then eats with those hands without washing them thoroughly, pinworm eggs pass through the person’s mouth and enter the gastrointestinal tract, causing self-infection. This can be. ● The habit of washing hands thoroughly is also important at this time. ● If the feces in the rectal tube are expelled with an enema, the mother pinworm may come out with the feces. ● Treatment in this way may temporarily reduce the number of pinworms in the intestinal tract, but pinworm disease cannot be fundamentally cured. ● Mebendazole has few side effects, is cheap, easy to take, and has good therapeutic effects. ● There is no need to treat with an enema or apply any medication to the anus. ● It is best to treat it by taking pinworm treatment drugs such as mebendazole according to the doctor’s prescription. ● (Refer to table 1-4 parasite infection treatment drugs on p.000) ■ Prevention of pinworm disease ● When someone in the family has pinworm disease, there is a high possibility that other family members also have pinworm disease. ● It is better not to treat only children with pinworm disease, but to treat all family members at the same time. ● However, some doctors say that it is better to treat only family members who have symptoms of pinworm disease or who have been confirmed to have pinworm disease. ● Pinworm eggs can infect the entire family, but they can also infect friends who play together or other children at daycare. ● Prevent pinworm disease by making it a habit to wash your hands thoroughly before eating.
