■ 이의 종류
● 이는 몸통에 주로 기생하는 몸이(옷이) 기생충증(Pediculosis corporis),
● 눈썹에 주로 기생하는 안검슬증(Pediculosis papebrarum),
● 몸통에 주로 기생하고 옷에 숨어사는 옷이 기생충증(몸이/Pediculosis vestimentorum),
● 머리에만 기생하는 머릿니 기생충증(두슬/Pediculosis capitis),
● 외부생식기 음모에 주로 기생하는 음모니 기생충증(사면발이/음모털이/Pediculosis pubis) 등이 있다.
● 과거에는 몸 이가 많이 유행됐지만 요즘에는 그렇게 흔치 않다.

● 세계 각처에서 사는 사람들이 서로 자주 왕래를 하기 때문에 이 기생충증을 세계 각처에서 찾아 볼 수 있다.
● 머릿니는 아직도 미국을 비롯해 세계 각 문명국 각처에서 사는 소아 청소년들과 성인들에게도 많이 발생되고 기생하고 있다.
■ 이에 물렸을 때의 증상 징후
● 이의 종류·이의 수·합병증 등에 따라 증상 징후가 다르다.
● 머릿니는 머리에만, 몸 이는 몸통에만, 음모 이는 외부생식기, 수염 또는 눈썹에 주로 기생한다.
● 머릿니가 기어 다니거나 물고 피를 빨아먹을 때 가렵다.
● 이 때 긁으면 피부에 상처가 생기고 부스럼이 나며 주위 림프절이 부을 수 있다.
● 머릿니는 머리의 어느 부위에서든지 기생할 수 있지만 뒤통수 부위에 더 많이 기생한다.
● 머릿니는 머릿니에 감염된 사람으로부터 다른 사람들에게로 직접 감염되거나 빗, 브러시, 가구 등을 통해서도 감염될 수 있다.
■ 이에 물렸을 때의 진단
● 병력과 증상 징후·진찰소견 등을 종합해 진단할 수 있다.
● 머리카락에 있는 머릿니의 알은 머리카락에 꼭 붙어서 쉽게 떨어지지 않는다.
● 이의 알을 서캐(사진 399-401 참조).
● 이 서캐는 육안으로도 볼 수 있다.
● 이가 머리에서 기어 다니는 것도 육안으로 볼 수 있다.
● 머릿니나 음부이는 흑갈색이고 몸 이는 누런 회백색이다.
● 몸 이는 옷의 맨 안쪽 면에 알을 낳는다.
■ 이에 물렸을 때의 치료
● 머릿니는 1% 린데인(Lindane) 성분이 든 쿠웰, 0.5% 마라티온(Malathion) 성분이 든 오비드(Ovide/Malathion성분),

● 또는 펄메스린(Permethrin) 성분이 든 닉스(Nix) 등으로 이 기생충증을 치료하고 이 기생충증 치료 약물 중 한 가지를 선택해 치료한다.
● 이 중 1% 린데인(Lindane) 성분이 든 쿠웰
은 성충도 죽일 수 있고 서케도 죽을 수 있다.
● Trimethoprim/sulfamethoxzole(Bacerim)을 경구로 복용하고 Permethrin으로 동시에 국소치료 하기도 하고, Ivermectin이나 Stromectol으로 치료도 한다
● 요즘 이런 이 기생충증 치료약으로 의사의 처방에 따라 치료해도 이 기생충증 치료약들에 내성이 생겨 이 기생충증을 치료하는데 어려움이 있다.
● 한 집안 식구들 중 누구 한 사람에게 머릿니나 몸이 기생충증이 있을 때는 이가 다른 식구들에게도 기생할 수 있으므로 전 식구가 같은 날에 같은 치료를 1차로 한다.
● 그 후 1주일 경 한 번 더 2차 치료한다.
● 쿠웰이나 오비드의 주성분은 파라치온이다.
● 이런 이 기생충증 치료약도 의사의 처방에 따라 써야 한다.
● 쿠웰·린덴·펄메스린·마라티온 중 한 가지의 머릿니 기생충증 치료약으로 머릿니를 치료할 때 이와 서캐가 다 죽을 수 있다.
● 이 치료약으로 치료를 한 후 죽은 서캐는 1~2주일 정도까지 머리카락에 계속 붙어있을 수 있다.
● 쿠웰·닉스·오비드·리드(Rid) 중 한 가지 머릿니 치료약으로 한 번 정도 치료하면 서캐가 다 죽는 것이 보통이다.
● 치료 후 머리카락에 계속 붙어있는 서캐는 30∼60분 동안 머리를 물수건 찜질을 한 후 서캐를 가는 빗으로 떼어 내든지, 머리카락에 백색 식초를 바른 후, 또는 백색 식초수건 찜질을 30∼60분 동안 한 후 서캐를 빗으로 떼어 내면 서캐가 더 쉽게 떨어진다.
● 입었던 옷이나 홑이불을 물에 삶아 빨고 침실은 깨끗이 청소한다.
● 옷이나 침대에 오염된 몸 이가 다른 사람에게 전염될 수 있다.
● 몸 이는 위생 관리가 불결하고 협소한 거주생활을 하는 소아 청소년들에게 기생할 가능성이 더 많다.
● 몸 이가 물고 피를 빨아먹을 때만 피부에 있고 나머지 시간은 속옷 맨 안쪽 면에 숨어 있고 또 거기에 알을 낳는다.
● 위생 관리를 철저히 하고 옷이나 홑이불을 물에 삶아 빨아 입어야 한다.
● 음모니(음부니)는 외부생식기에 주로 기생한다.
● 1% 린데인(Lindane) 성분이 든 쿠웰 로숀으로 치료하면 성충도 죽일 수 있고 서케도 죽을 수 있다
● 드물게 겨드랑이 털, 눈썹, 수염이 난 얼굴 피부에도 기생할 수 있다.
● 머릿니를 치료할 때 쓰는 이 기생충증 치료약으로 치료하고 서캐는 가는 빗으로 빗어 떼어낸다.
● 음모니가 옷이나 홑이불에 때로는 오염될 수 있다.
● Permethrin 성분이 든 PerForm 이기생충약이 최근 미 Formix Ltd에서 제조했다.
● 이 머릿니 기생충증 치료의 효과가 96%정도 된다고 한다.
● DSP 로션 기생충약이 최근 미국에서 제조됐는데 머릿니 기생충증 치료효과가 97%정도라고 한다.3333333333333333333333
● Benzyl alcohol lotion으로 머릿니 감염증을 치료할 수 있다(출처; Physician First watch, April 2009).

며칠 전 7살 딸아이 머리를 빗기는데 머리카락에 뭔가가 달려있는걸 보고 혹시나 했는데, 역시더군요. 이가 생긴 거였습니다. 그동안 가렵다고 호소하지 않았기에 전혀 몰랐습니다. 며칠 동안 남에게 얘기도 못하고 끙끙거리며 참빗을 온 동네를 샅샅이 뒤져 사다가 빗기고 서캐도 뽑고 했죠.
저도 아무래도 옮은 것 같은데 선생님, 샴푸형태로 생긴 약이 있다고 인터넷에서 보았는데, 어찌하면 좋을까요? 아이야 제가 책임지고 박멸한다고 해도 중이 제 머리 못 깎는다고 제 머리는 아무래도 약을 써야겠기에 이렇게 의논드립니다. 제발 도와주세요…
만약 병원을 가게 된다면 무슨 과를 찾아야 하는지요? 병원은 죽어도 가기 싫은데, 너무너무 창피합니다.

● 안녕하세요. 질문해 주셔서 감사합니다. 좋은 질문입니다.
● 자녀의 나이, 성별, 과거 병력, 가족 병력, 진찰소견, 임상검사 등의 정보를 많이 알수록 답을 드리는데 도움이 됩니다. 주신 정보를 토대로 해서 답변을 드리겠습니다.
● 이에는 머릿니, 몸이, 음모니 등이 있습니다. 미국에서도 머릿니 때문에 초 중학생들이 학교에서 등교를 거부당하고 머릿니 기생충증을 치료하는데 많은 문제가 생기기도 했습니다. 머릿니 감염증이 있으면 다른 학생들에게 전염 될 수 있기 때문에 머릿니 감염증이 완치되지 않은 상태에 있는 학생은 등교할 수 없습니다.
● 치료에는 Permethin(1%) 크림, Pyrethrin-based products 샴푸, Lindane (1%) 샴푸, Malathion (0.5%) 로션 등 여러 종류의 이 감염증 치료약들 중 한 종류를 선택해서 치료할 수 있으나 Malathion(상품명-Ovide) 로션이 가장 치료효과가 좋습니다.
● 이런 이 감염증 치료약으로 의사의 지시에 따라 잘 치료하면 이 알(서캐)도 죽지만 이 알껍데기(▴ 사진 401 참조)가 1~2주 동안 머리카락에 그냥 붙어 있기 때문에 보기에도 좋지 않고 때로는 학교에서 완치가 되지 않았다고 진단하고 등교를 거부하는 경우도 있습니다. 때문에 이 같은 염증 치료약으로 치료한 후에 서캐는 가는 빗으로 빗어 제거해야 쉽게 없어집니다.
● 요즘 이 감염증 약물치료에 이가 죽지 않기 때문에 이 감염증이 잘 치료되지 않는 경우도 있습니다. 일반적으로 온 식구들이 같은 날 같은 치료를 받고 또 가능한 한 가까운 친구들도 치료를 받아야 할 때도 있습니다.
● 그리고 침구, 옷 등을 깨끗이 빨고 애완동물도 동시 치료해 주는 것이 좋습니다.
● 1차 치료를 받은 후에 2일~1주일 내에 같은 이 감염증 치료약물로 다시 한 번 2차 치료를 하십시오.
● 소아청소년과에서 계속 치료 받으시고 이런 문제에 관해서 상담하시기 바랍니다.

Choose one of these medications to treat lice. ● Of these, Quwell, which contains 1% Lindane, can kill both adult lice and nits. ● Trimethoprim/sulfamethoxazole (Bacerim) is taken orally, and topical treatment with Permethrin is also used. Ivermectin or Stromectol is also used. ● Even with these current anti-lice medications prescribed by a doctor, resistance to them has made it difficult to treat lice. ● If one member of a household has head lice or body parasites, the lice can spread to other family members. Therefore, the entire household should receive the same treatment on the same day. ● A second treatment is then administered approximately one week later. ● The active ingredient in Quwell and Ovid is parathion. ● These anti-lice medications should also be used as prescribed by a doctor. ● Treating head lice with one of Quwell, Lindane, Permethrin, or Marathion will kill both the lice and the nits. ● After treatment with this medication, dead nits may remain in the hair for up to one to two weeks. ● One treatment with one of the following head lice medications usually kills all nits. ● For nits still attached to the hair after treatment, apply a wet towel to the hair for 30 to 60 minutes and then remove the nits with a fine-tooth comb. Alternatively, apply white vinegar to the hair, or apply a white vinegar towel to the hair for 30 to 60 minutes and then remove the nits with a comb. This will help the nits fall out more easily. ● Wash clothes and bedding after washing them in water, and thoroughly clean the bedroom. ● Body lice contaminated with clothing or bedding can be spread to others. ● Body lice are more likely to infest children and adolescents who live in cramped quarters or poor hygiene. ● Body lice only live on the skin when biting and sucking blood. What should I do? Even if I take responsibility for eradicating them, my child won’t be able to shave my head, so I’m asking this question because I’ll have to use medication. Please help me… If I were to go to the hospital, what department should I go to? I hate going to the hospital more than anything, and it’s so embarrassing. Thank you for your hard work. A. To a Worried Mom ● Hello. Thank you for your question. It’s a good question. ● The more information I have about your child’s age, gender, past medical history, family medical history, physical findings, and clinical tests, the more helpful it is to answer your question. I will answer based on the information you provided. ● Lice include head lice, body lice, and pubic lice. In the United States, elementary and middle school students have been refused school due to head lice, and treatment for head lice parasitemia has been problematic. Because head lice can spread to other students, students who have not been cured are not allowed to attend school. ● Treatment can be achieved with a variety of anti-lice medications, including Permethin (1%) cream, Pyrethrin-based shampoos, Lindane (1%) shampoo, and Malathion (0.5%) lotion. However, Malathion (brand name: Ovide) lotion is the most effective. ● Proper treatment with these anti-lice medications, as directed by a doctor, will kill the lice eggs (nits). However, the lice eggshells (see Photo 401) remain attached to the hair for 1-2 weeks, which is unsightly. In some cases, schools may diagnose the lice as incurable and refuse to attend school. Therefore, after treatment with these anti-lice medications, the nits should be combed out with a fine-toothed comb to easily remove them. ● Because current anti-lice medications do not kill lice, some lice infections are not cured effectively. Typically, the entire family receives the same treatment on the same day, and sometimes even close friends should be treated as well. ● It’s also recommended to wash bedding, clothes, etc. thoroughly and treat pets at the same time. ● After the first treatment, administer a second round of treatment with the same medication for this infection within 2 days to 1 week. ● Please continue receiving treatment at a pediatrician and consult with your child’s doctor regarding this issue. ● Please refer to the lice infection section. ● If you have any further questions, please contact us again. ● Thank you.
