■ Causes of Ascariasis
Ascariasis, a parasitic infection caused by the roundworm (Ascaris lumbricoides), is called ascariasis or macroembryonia.
Ascariasis exists in both male and female forms, and they vary in size (Photo 5-3).
A mature male ascaris is 15–25 cm long, while a mature female is 25–35 cm long, with a body diameter of 3–6 mm.
The parasite is slender, long, and round, similar to an earthworm.
Male and young ascaris are much smaller than the mother female.
Ascariasis primarily inhabits the small intestine.
Ascaris eggs enter the mouth through contaminated beverages, food, or hands, where they develop into parasites in the small intestine.
■ Symptoms and Signs of Ascariasis

● Ascaris eggs, which enter the small intestine through the oral cavity, hatch and develop into larvae.
● When these larvae migrate to the respiratory tract, they can cause aspiratory wheezing, coughing, and allergic pneumonia.
● The symptoms and signs of Ascariasis vary significantly depending on the number of Ascaris worms living in the small intestine and the location of the parasites in the body.
● When a very small number of Ascaris worms live in the small intestine, few or no symptoms may be present.
● When the number of roundworms in the small intestine increases significantly, severe abdominal pain and vomiting may occur.
● Bile-tinged gastrointestinal fluid may be vomited, and sometimes the entire worm may be vomited.
● Steatorrhea may also occur.
● Roundworms may clump together, forming a mass that can obstruct the small intestine.
● When vomiting or coughing due to certain infectious diseases causes the worms in the gastrointestinal tract to be expelled through the mouth or nose.
● Without treatment and without any symptoms, the worms may spontaneously pass out with the stool. These worms can migrate beyond the gastrointestinal tract to other organs in the abdominal cavity, causing various helminthic diseases such as peritonitis, biliary atresia, and appendicitis.
● When larval roundworms penetrate the intestinal wall and migrate through the bloodstream to other parts of the body, allergic pneumonia due to roundworm infestation can occur.
● Ascariasis can cause hives, bronchial asthma, and various other ailments.
● In addition to the symptoms described above, other symptoms may include abdominal pain, loss of appetite, weight loss, insomnia, low-grade fever, nervousness, malnutrition, and dysphagia.
● When roundworm larvae migrate within the body, eosinophilia and Löffler syndrome can also occur.
■ Diagnosis and Treatment of Ascariasis
● If ascariasis is suspected based on a combination of medical history, symptoms, and physical examination, the diagnosis can be made by detecting the mother worm or eggs in the stool.
● In rare cases, the initial diagnosis of ascariasis can be made by observing the mother worm in the stool or mouth.
● Taking 100 mg of mebendazole (Vermax) twice daily for three days can lead to a nearly 100% cure. A second round of mebendazole treatment is administered one week after the first round of treatment.
● Mebendazole treatment is not recommended for infants and children under two years of age.
● Instead, a single dose of pyrantel pamoate (11 mg per kg of body weight) is recommended.
● Piperazine is said to be effective in treating ascariasis.
● Albendazole is sometimes used, but pregnant women should not use this medication.
● Grinding papaya seeds, mixing one teaspoon with sugar water, and taking it once a week for two weeks can treat parasitic infections caused by roundworms, hookworms, and whipworms (p.00 Table 1-4 Parasitic Infection Treatment).
■ Prevention of Ascariasis
● Only female roundworms can lay eggs.
● Eggs laid by the mother roundworm in the small intestine can also be expelled from the body with the feces.
● Eating unwashed or undercooked vegetables contaminated with eggs from the stool can cause ascariasis.
● Touching soil or toilets contaminated with eggs with bare hands and then eating food without washing your hands can cause ascariasis, ascariasis.
● Using human waste as fertilizer can also spread ascariasis eggs to others.
● Allowing human waste to decompose for a sufficient period of time will kill the eggs.
● Therefore, only use human waste with dead eggs as fertilizer.
● Vegetables contaminated with eggs should be thoroughly washed with water before eating.
● Wash your hands thoroughly before eating, especially immediately before eating.
● Clean toilets thoroughly to prevent ascariasis.

Three weeks ago, the whole family took deworming medication, but my second son found a long roundworm in his stool.
Two of them were found.

● Hello. Thank you for your question. It’s a good question.
● The more information we have about your child’s age, gender, past medical history, family medical history, physical examination findings, and clinical tests, the more helpful it is for me to answer your question. I will provide an answer based on the information you provided.
● Congratulations on your country’s great victory in the World Cup.
● If roundworms are found, you can treat your baby with deworming medication again, perform another stool parasite test, or test the entire family for stool parasites.
● However, since a stool parasite test can produce a false negative result if parasites are still present, it may be better to have the entire family take deworming medication again without performing a stool test.
● It’s also important to identify where the parasites originated and prevent future roundworm infections.
