■ 회충증의 원인

회충(Ascaris lumbricoides) 감염으로 생기는 기생충증을 회충증 또는 거시배라고 한다.

회충에는 암컷과 수컷이 있고 암컷과 수컷의 크기가 다르다(사진 5-3).

성숙한 수컷 회충의 길이는 15∼25cm, 암컷은 25∼35cm 정도 길고, 몸통의 직경이 3∼6mm 정도 된다.

지렁이와 같이 가늘고 길고 둥근 원형으로 생긴 기생충이다.

수컷 회충이나 새끼 회충은 어미 암컷 회충보다 훨씬 작다.

회충은 작은창자 속에서 주로 기생한다.

회충 알이 오염된 음료수, 음식물, 또는 손을 통해서 회충 알이 경구로 들어와 소장관 속에서 회충이 기생한다.

■ 회충증의 증상 징후

● 경구를 통해서 소장관 속으로 들어온 회충 알이 깐 후 회충의 유충이 된다.

● 이 유충이 호흡기 내로 이주할 때 호기성 천명이 생기고 기침을 하면서 알레르기성 폐렴이 생길 수 있다.

● 소장관 속에서 기생하는 회충의 수에 따라, 또 신체 어느 부위에 기생하고 있느냐에 따라 회충증의 증상 징후가 많이 다르다.

● 극소수의 회충이 소장관 속에서 기생할 때는 증상 징후가 거의 없거나 비특이 증상 징후가 생길 수 있다.

● 소장관 내에서 회충의 수가 상당히 많아질 때는 심한 복통이 생길 수 있고 구토를 할 수 있다.

● 담즙이 섞인 위장 액을 토할 수 있고 때로는 어미 회충을 통째로 토하기도 한다.

● 지방변증도 생길 수 있다.

● 회충들이 서로 엉겨 붙어서 회충 뭉치가 되고 그 뭉치로 소장관이 폐쇄될 수 있다.

● 어떤 감염병으로 구토를 하거나 또는 기침을 할 때 위장관 속에 있던 어미 회충이 입이나 코를 통해서 입 밖으로 나올 수 있다.

● 아무런 치료를 해 주지 않고 증상 징후가 없는데 어미 회충이 대변과 같이 자연히 나올 수 있다. 회충이 위장관 속 이외 복강내 다른 장기 속으로 이주하여 복막염·담도폐색증·충수염 등 여러 가지 회충병을 일으킬 수 있다.

● 회충의 새끼벌레가 장관 벽을 뚫고 혈류를 따라 신체 다른 부위로 이주할 때 회충 알레르기성 폐렴이 생길 수 있다.

● 회충증이 있을 때 두드러기, 기관지 천식이나 그 밖의 다른 여러 가지 병을 일으킬 수 있다.

● 앞에서 설명한 증상 징후 이외 복통, 식욕감퇴, 체중감소, 불면증, 미열, 신경질, 영양장애, 이미증 등의 증상 징후가 생길 수 있다.

● 회충의 유충이 체내에서 이행할 때 호산구증과 뢰플러 증후군도 생길 수 있다.

■ 회충증의 진단 치료

● 병력·증상·진찰소견을 종합해서 회충증이 의심되면, 대변에서 어미 회충이나 회충알을 검출해서 회충증을 진단할 수 있다.

● 드물지만 대변이나 입 안으로 나온 어미 회충을 보고 회충증이 있는지 처음 진단할 때도 있다.

● 메벤다졸(Mebendazole/Vermax) 100mg을 1일 2회 3일간 복용하면 거의 100% 완치될 수 있다. 1차 치료를 하고 1주 후에 같은 방법으로 메벤다졸로 2차 치료를 한다.

● 2세 이전 영유아들은 메벤다졸로 치료받는 것을 권장하지 않는다.

● 그 대신 피란텔 파모에이트(Pyrantel pamoate)를 체중 1kg 당 11mg을 1회 용량으로 계산해서 1회 복용 치료한다.

● 피페라진(Piperazine) 약물로 회충증을 치료하면 좋은 치료 효과가 있다고 한다.

● 때로는 Albendazole제로 치료하기도 하나 임신부는 이 약으로 치료해서는 안 된다.

● 파파야씨를 갈아서 한 티스푼을 설탕물에 섞어 1주에 1회, 총 2주 복용하면 회충, 십이지장충, 편충으로 생긴 기생충증이 치료될 수 있다고 한다(p.00 표1-4 기생충 감염 치료).

■ 회충증의 예방

● 암컷 어미 회충만이 알을 낳을 수 있다.

● 어미 회충이 소장관 속에서 낳은 회충 알이 대변과 같이 몸 밖으로 나오기도 한다.

● 대변에 있던 회충 알에 오염된, 채소를 깨끗이 씻지 않거나 익히지 않은 채로 먹을 때 회충증에 걸릴 수 있다.

● 회충 알에 오염된 흙이나 변기 등을 맨손으로 만진 후에 손을 씻지 않고 음식물을 먹을 때 회충 알이 입을 통하여 몸속으로 들어가서 회충증에 걸릴 수 있다.

● 인분을 거름으로 사용할 때도 회충 알이 다른 사람에게 감염될 수 있다.

● 충분한 기간 동안 인분을 썩히면 회충 알이 죽는다.

● 그 때문에 회충 알이 죽은 인분만 거름으로 써야 한다.

● 회충 알에 오염된 채소 등은 물로 잘 씻은 후 먹어야 한다.

● 특히 식사하기 바로 전에는 손을 깨끗이 씻는 습관을 가져야 한다.

● 변기를 깨끗이 청소해서 회충증을 예방해야 한다. .

● 회충이 나왔으면 아기에게 구충제로 한 번 더 치료하든지 또는 대변 기생충 검사를 한 번 더 하든지, 또는 온 식구들의 대변 기생충 검사를 하는 방법도 있습니다.

● 그러나 기생충이 체내에 있으면서 대변 기생충 검사의 결과가 허위 음성으로 나타날 수 있으므로 변 검사를 하지 않고 온 식구가 구충제를 다시 한 번 복용하는 것도 좋을 것입니다.

● 어디에서 기생충이 감염됐는지 알아 회충증에 또 다시 걸리지 않게 예방하는 것도 중요합니다.

■ Causes of Ascariasis
Ascariasis, a parasitic infection caused by the roundworm (Ascaris lumbricoides), is called ascariasis or macroembryonia.
Ascariasis exists in both male and female forms, and they vary in size (Photo 5-3).
A mature male ascaris is 15–25 cm long, while a mature female is 25–35 cm long, with a body diameter of 3–6 mm.
The parasite is slender, long, and round, similar to an earthworm.
Male and young ascaris are much smaller than the mother female.
Ascariasis primarily inhabits the small intestine.
Ascaris eggs enter the mouth through contaminated beverages, food, or hands, where they develop into parasites in the small intestine.
■ Symptoms and Signs of Ascariasis

● Ascaris eggs, which enter the small intestine through the oral cavity, hatch and develop into larvae.

● When these larvae migrate to the respiratory tract, they can cause aspiratory wheezing, coughing, and allergic pneumonia.

● The symptoms and signs of Ascariasis vary significantly depending on the number of Ascaris worms living in the small intestine and the location of the parasites in the body.

● When a very small number of Ascaris worms live in the small intestine, few or no symptoms may be present.

● When the number of roundworms in the small intestine increases significantly, severe abdominal pain and vomiting may occur.

● Bile-tinged gastrointestinal fluid may be vomited, and sometimes the entire worm may be vomited.

● Steatorrhea may also occur.

● Roundworms may clump together, forming a mass that can obstruct the small intestine.

● When vomiting or coughing due to certain infectious diseases causes the worms in the gastrointestinal tract to be expelled through the mouth or nose.

● Without treatment and without any symptoms, the worms may spontaneously pass out with the stool. These worms can migrate beyond the gastrointestinal tract to other organs in the abdominal cavity, causing various helminthic diseases such as peritonitis, biliary atresia, and appendicitis.

● When larval roundworms penetrate the intestinal wall and migrate through the bloodstream to other parts of the body, allergic pneumonia due to roundworm infestation can occur.

● Ascariasis can cause hives, bronchial asthma, and various other ailments.

● In addition to the symptoms described above, other symptoms may include abdominal pain, loss of appetite, weight loss, insomnia, low-grade fever, nervousness, malnutrition, and dysphagia.

● When roundworm larvae migrate within the body, eosinophilia and Löffler syndrome can also occur.

■ Diagnosis and Treatment of Ascariasis

● If ascariasis is suspected based on a combination of medical history, symptoms, and physical examination, the diagnosis can be made by detecting the mother worm or eggs in the stool.

● In rare cases, the initial diagnosis of ascariasis can be made by observing the mother worm in the stool or mouth.

● Taking 100 mg of mebendazole (Vermax) twice daily for three days can lead to a nearly 100% cure. A second round of mebendazole treatment is administered one week after the first round of treatment.

● Mebendazole treatment is not recommended for infants and children under two years of age.

● Instead, a single dose of pyrantel pamoate (11 mg per kg of body weight) is recommended.

● Piperazine is said to be effective in treating ascariasis.

● Albendazole is sometimes used, but pregnant women should not use this medication.

● Grinding papaya seeds, mixing one teaspoon with sugar water, and taking it once a week for two weeks can treat parasitic infections caused by roundworms, hookworms, and whipworms (p.00 Table 1-4 Parasitic Infection Treatment).

■ Prevention of Ascariasis

● Only female roundworms can lay eggs.

● Eggs laid by the mother roundworm in the small intestine can also be expelled from the body with the feces.

● Eating unwashed or undercooked vegetables contaminated with eggs from the stool can cause ascariasis.

● Touching soil or toilets contaminated with eggs with bare hands and then eating food without washing your hands can cause ascariasis, ascariasis.

● Using human waste as fertilizer can also spread ascariasis eggs to others.

● Allowing human waste to decompose for a sufficient period of time will kill the eggs.

● Therefore, only use human waste with dead eggs as fertilizer.

● Vegetables contaminated with eggs should be thoroughly washed with water before eating.

● Wash your hands thoroughly before eating, especially immediately before eating.

● Clean toilets thoroughly to prevent ascariasis.

● The more information we have about your child’s age, gender, past medical history, family medical history, physical examination findings, and clinical tests, the more helpful it is for me to answer your question. I will provide an answer based on the information you provided.

● Congratulations on your country’s great victory in the World Cup.

● If roundworms are found, you can treat your baby with deworming medication again, perform another stool parasite test, or test the entire family for stool parasites.

● However, since a stool parasite test can produce a false negative result if parasites are still present, it may be better to have the entire family take deworming medication again without performing a stool test.

● It’s also important to identify where the parasites originated and prevent future roundworm infections.
