● 현저한 비타민 D 결핍증은 드물지만 무증상 비타민 D 결핍증은 비교적 흔하다.
● 최근 미 알라스카주, 아이오와주, 네바다주 등 각주의 영유아들에게 비타민 D 결핍증과 구루병(Rickets)의 발생률이 증가되고 있다고 한다.
● 그뿐만 아니라 영국을 비롯한 전 세계 각처의 영유아들에게 비타민 D 결핍증과 구루병(Rickets) 발생률이 증가하고 있다.
● 중국 12~24개월 유아의 65.3%에서 비타민 D 결핍증과 구루병(Rickets)이 발생했다고 한다. (소스; Arch pediatrics adolescence med. June 2008. P505)

■ 비타민 D의 기능
● 비타민 D는 경구를 통해 섭취된 칼슘과 인(P)이 위장관 벽 점막층을 통해 혈관 속으로 적절히 흡수되도록 촉진하는 역할을 한다.
● 혈중 인의 농도가 비정상적으로 높을 때 필요 이상의 인이 신장을 통해 소변으로 배설되도록 촉진하는 역할도 있다.
● 그래서 뼈의 주성분인 칼슘과 인의 혈중 농도와 체내 총량을 수요공급 원칙에 따라 적절하게 조절하는 역할을 한다.
● 인체에서 인이 더 필요할 때는 신장이 인을 소변으로 덜 배설하게 한다.
● 인체에서 인이 덜 필요할 때, 인이 신장에서 더 많은 양이 배설되도록 촉진하는 작용도 한다.

■ 비타민 D의 공급과 비타민 D 결핍이 생기는 경우
● 비타민 D는 자연산 음식물의 재료, 비타민을 첨가한 음식물, 비타민 D가 든 종합 비타민, 모유, 약물 등에서 섭취할 수 있다.
● 피부가 햇볕에 노출될 때 비타민 D가 생성된다.
● 비타민 D가 들어 있지 않은 음식물이나 극소량 들어 있는 음식물을 장기간 섭취할 때 비타민 D 결핍증이 생길 수 있다.

● 피부가 햇볕에 많이 노출될 수 없는 북극 지방에서 장기간 사는 소아청소년들에게 비타민 D 결핍증이 생길 수 있다.
● 체중이 아주 작게 태어난 미숙 신생아들, 아주 짧은 기간 동안 아주 빠른 성장 속도로 자라는 영유아들, 또는 사춘기 아이들에게 비타민 D 결핍증이 생길 수 있다.
● 비타민 D 결핍증이 장기간 계속되면 구루병(Rickets)에 걸릴 수 있다.
● 장기간 설사를 하는 아이들이나 췌장염, 간염, 신장염 등을 앓는 아이들에게 비타민 D 결핍증, 또는 구루병이 생길 수 있다.
● 대구 간유나 달걀노른자, 또는 비타민 D를 첨가한 전우유, 분유, 인공영양 등 음식물에는 비타민 D가 많이 들어 있다.
● 생우유에는 비타민 D가 소량 함유되어 있다.
● 균형 잡힌 음식을 충분히 섭취하는 수유모의 젖 속에는 젖먹는 아기에게 필요한 만큼의 비타민 D가 들어 있다.
● 피부가 햇볕의 자외선에 노출될 때 피부층에 있는 프로비타민 D가 비타민 D로 변한다.
● 자외선은 유리창문을 통과해서 실내로 들어올 수 없다.
● 햇볕이 유리 창문을 통과해서 실내로 들어올 수는 있지만 햇볕 속 자외선은 유리 창문을 통과할 수 없기 때문에 유리 창문 안 실내에서 주로 일상생활을 장기간 하는 사람들의 피부에 있는 프로비타민 D는 비타민 D로 변화될 수 없다.
● 따라서 이런 종류의 실내에서 주로 거주하는 소아청소년들이나 성인들에게는 비타민 D 결핍증이 생길 수 있다.
● 요즘 다세대 주택이나 아파트에서 많은 시간을 주로 보내는 영유아들에게 비타민D 결핍증이 생길 수 있다.
● 몸에 필요한 비타민 D를 충분이 만들 수 없을 정도로 햇볕에 충분히 노출될 수 없는 지방이나 장소에서 사는 사람들, 햇볕에 노출 될 수 있는 기회가 적은 북극 지방에서 사는 사람들은 자외선에 충분히 노출 되지 않기 때문에 피부에서 비타민 D가 충분히 만들어지지 않기 때문에 비타민 D 결핍증이 생길 수 있다.
● 특히 북극 지방에 사는 사람들은 보통 먹는 음식물에서 비타민 D를 충분히 섭취해서 구루병을 예방해야 한다.
● 영유아들은 햇볕을 틈틈이 쏘여 비타민 D 결핍증을 예방해 주는 것이 좋다.
● 그러나 북극 지방 같은 특수한 지방에서 살지 않는 한 추운 철에 어린 영유아들을 일부러 집 밖으로 데리고 나가 햇볕에 노출시킬 필요는 없다.
● 북극지방에 사는 사람들, 특히 위도 40도 이상 북쪽 지방에서 사는 사람들, 흑인들, 비타민 D가 충분히 들어 있지 않은 모유를 수유하는 아기들에게 비타민 D 결핍증이 생기지 않게 예방하기 위해 비타민 A D제를 복용하는 것이 좋다.
● 피부에서 비타민 D가 충분히 만들어지는지 확실히 확인하는 방법이 별로 없다.
● 인공영양에 비타민 D가 충분히 포함되어 있어 인공영양을 먹는 영아들에게 비타민 D를 추가로 더 줄 필요가 없다.

■ 비타민 D 결핍증과 구루병의 증상 징후
● 비타민 D 결핍증이 있는 임산부에게 태어난 아기를 비타민 D제로 적절히 치료하지 않으면 생후 2~3개월 내에 비타민 D 결핍증이 생길 수 있다.
● 영아들이 오랫동안 비타민 D를 충분히 섭취하지 못하면 구루병이 서서히 생기기 시작하다가 1~2세경에 그 증상 징후가 현저히 나타날 수 있다.
● 구루병이 있으면 뼈가 약해지고 신체 발육이 지연되어 비정상적으로 늦게 앉고 걷는 발육이 지연될 수 있다.
● 구루병이 있는 영유아의 양손을 잡고 두 발로 서 있게 할 때, 두 다리가 비정상적으로 X형각이나 O형각으로 구부러질 수 있고 등뼈가 꼽추처럼 구부러질 수 있다.
● 두개골이 연약하고 머리의 모양이 비정상적으로 불균형한 형태로 될 수 있다.
● 앞쪽 늑골뼈들의 연골 끝 부분이 서로 연결되는 늑골 연골 접합 부분들이 두드러지게 커진다.
● 비타민 D가 든 종합 비타민제나 종합 비타민 젤을 첨가해 만든 인공영양을 먹이고 각종 비타민을 첨가한 이유식 등을 동시에 먹고 균형 잡힌 음식물을 섭취하고 햇볕을 적절히 쏘이면 영유아들에게 비타민 D 결핍증이 거의 생기지 않는다.
● 그러나 요즘 흙색 피부를 가진 유아들, 비타민 D를 엑스트라로 먹지 않는 아이들, 전적으로 모유만 먹는 영유들, 가속도로 성장하는 영유아들, 학령기 아이들과 사춘기의 아이들에게 비타민 D 결핍증이 생길 수 있다.
● 뼈 X선 검사에서 구루병으로 인한 특이한 변화를 볼 수 있다.
● 때로는 너무 많이 섭취해서 비타민 D 과다증에 걸릴 수 있다.
● 췌장 부전증, 세일리악병(Celiac disease), 크론병, 또는 신장 부전증이 있으면 비타민 D 결핍증이 생길 수 있다.

■ 비타민 D 결핍증과 구루병의 진단
● 병력 증상 징후 진찰소견 등을 종합하여 이병이 의심되면 혈청 25 OHD 농도를 검사해 진단한다.
● 혈청 25 OHD 농도가 20 ng/mL 이하이고 뼈 X선 사진검사에 나타난 특이 현상을 참작해 진단한다.

■ 비타민 D 결핍증과 구루병의 치료
● 비타민 D 결핍증의 원인에 따라 치료한다.
● 비타민 D나 비타민 D가 든 종합 비타민제로 치료한다.
● 그리고 균형 잡힌 음식물을 먹고 햇볕을 적절히 쏘인다.

● While overt vitamin D deficiency is rare, asymptomatic vitamin D deficiency is relatively common.
● Recently, the incidence of vitamin D deficiency and rickets (Rickets) has been increasing in infants and toddlers in Alaska, Iowa, Nevada, and other states in the United States.
● Furthermore, the incidence of vitamin D deficiency and rickets (Rickets) is increasing in infants and toddlers worldwide, including in the UK.

■ Functions of Vitamin D
● Vitamin D facilitates the proper absorption of calcium and phosphorus (P) taken orally into the bloodstream through the mucosal layer of the gastrointestinal tract.
● When blood phosphorus levels are abnormally high, vitamin D also promotes the excretion of excess phosphorus through the kidneys and urine.
● Therefore, vitamin D regulates the blood concentrations and total body amounts of calcium and phosphorus, the main components of bone, according to the principles of supply and demand.
● When the body needs more phosphorus, vitamin D promotes the excretion of less phosphorus through the kidneys and urine.
● When the body needs less phosphorus, vitamin D also promotes the excretion of more phosphorus through the kidneys.
■ Vitamin D Supply and Cases of Vitamin D Deficiency
● Vitamin D can be obtained from natural foods, foods with added vitamins, multivitamins containing vitamin D, breast milk, and medications. ● Vitamin D is produced when the skin is exposed to sunlight.
● Vitamin D deficiency can occur when the skin consumes foods that do not contain vitamin D or foods with very low amounts over a long period of time.

● Vitamin D deficiency can occur in children and adolescents who live long periods in arctic regions where skin is limited to sunlight.
● Premature infants born with very low birth weight, infants who grow very rapidly in a short period of time, and adolescent children can develop vitamin D deficiency.
● Long-term vitamin D deficiency can lead to rickets. ● Children with prolonged diarrhea or those with pancreatitis, hepatitis, or nephritis may develop vitamin D deficiency or rickets.
● Foods such as cod liver oil, egg yolks, and whole milk, formula, and formula with vitamin D added are rich in vitamin D.
● Raw cow’s milk contains small amounts of vitamin D.
● Breastfeeding mothers who eat a balanced diet can provide the necessary amount of vitamin D for their babies.
● When the skin is exposed to ultraviolet rays from sunlight, provitamin D in the skin layer is converted to vitamin D.
● UV rays cannot pass through glass windows.
● While sunlight can pass through glass windows, the UV rays in sunlight cannot penetrate them. Therefore, provitamin D in the skin of people who spend long periods of time indoors behind glass windows cannot be converted to vitamin D.
● Therefore, children, adolescents, and adults who spend a lot of time indoors in these types of environments may develop vitamin D deficiency. ● Infants and toddlers who spend a lot of time in multi-family homes or apartments these days are at risk of developing vitamin D deficiency.
● People who live in areas or places where sunlight exposure is insufficient to produce sufficient vitamin D, such as those in arctic regions with limited sunlight exposure, may develop vitamin D deficiency because their skin does not produce enough vitamin D due to insufficient exposure to UV rays.
● People living in arctic regions, in particular, should ensure adequate vitamin D intake through their diet to prevent rickets.
● Infants and toddlers should ensure adequate sunlight exposure to prevent vitamin D deficiency.
● However, unless you live in a special region like the arctic, there is no need to expose young children to sunlight during the cold season.
● People living in arctic regions, particularly those north of 40 degrees latitude, African Americans, and breastfed infants, who often do not have sufficient vitamin D, are advised to take vitamin D supplements to prevent vitamin D deficiency. There are few ways to determine for sure whether the skin is producing enough vitamin D.
Since the formula contains sufficient vitamin D, supplementation is not necessary for formula-fed infants.
Signs and Symptoms of Vitamin D Deficiency and Rickets
Babies born to mothers with vitamin D deficiency can develop vitamin D deficiency within the first 2-3 months of life if not properly treated with vitamin D supplements.
If infants do not receive sufficient vitamin D for a long time, rickets can develop gradually, with the symptoms becoming noticeable around 1-2 years of age.
Rickets can cause weak bones and delayed physical development, leading to abnormally delayed sitting and walking.
When infants with rickets are held by both hands and held on both feet, their legs may bend abnormally in an X- or O-shaped angle, and their spine may become hunched.
The skull may be fragile, and the head may be abnormally shaped and disproportionate. ● The costochondral joints, where the ends of the anterior ribs connect to the ends of the costal cartilage, become noticeably enlarged.
● Vitamin D deficiency is rare in infants and young children when they are fed formula supplements containing vitamin D, such as multivitamins or multivitamin gels, along with baby food supplemented with various vitamins, a balanced diet, and adequate sunlight exposure.
● However, vitamin D deficiency can occur in infants with dark skin, those who do not receive supplemental vitamin D, those who are exclusively breastfed, infants and young children who are growing rapidly, school-aged children, and children going through puberty.
● Bone X-rays can show unusual changes associated with rickets.
● Sometimes, excessive intake can lead to vitamin D overdose.
● Pancreatic insufficiency, celiac disease, Crohn’s disease, or kidney failure can also cause vitamin D deficiency.
■ Diagnosis of Vitamin D Deficiency and Rickets
● If this condition is suspected based on a comprehensive review of medical history, symptoms, signs, and physical findings, a serum 25 OHD concentration test is performed to diagnose the condition.
● A serum 25 OHD concentration of 20 ng/ml or lower and any unusual findings on a bone X-ray are considered for diagnosis.
■ Treatment of Vitamin D Deficiency and Rickets
● Treatment depends on the cause of vitamin D deficiency.
● Treatment involves vitamin D or a multivitamin containing vitamin D.
● In addition, patients should eat a balanced diet and get adequate sunlight exposure.
Vitamin D deficiency increases the risk of fractures.
