■ 코 외상과 코뼈(비골) 골절의 원인, 증상 징후
● 코는 안면에서 가장 두드러지게 앞으로 나와 있기 때문에 안전사고 등으로 얼굴을 어디에 부딪치면 제일 먼저 코가 다칠 수 있다.
● 코에 생긴 외상의 정도, 원인, 그때 생긴 다른 외상의 유무 등에 따라 증상 징후가 다르다.
● 코에 타박상을 심하게 입을 때는 코가 붓고 아프면서 코피가 나고 코뼈가 골절될 수 있다.
● 코뼈는 위로 경골이 있고 그 아래의 코뼈는 연골이 있다(그림 156 참조).
● 금만 간 경미한 코뼈 골절, 골절된 코뼈의 일부가 비강 내로 쑥 밀려들어간 함몰 코뼈 골절, 골절된 코뼈의 일부가 코의 한쪽으로 밀려들어간 코뼈 골절 등 여러 종류의 코뼈 골절이 생길 수 있다.

● 코의 비강은 비중격 뼈로 오른쪽 비강과 왼쪽 비강으로 나누어진다.
● 비중격이 골절되면 한쪽 비강 속이 다른 쪽 비강 속보다 더 좁아져서 좁은 쪽 비강 속이 쉽게 막힐 수 있다.
● 이때 막힌 비강을 통하여 숨 쉬기가 곤란해진다.

■ 코 외상과 코뼈(비골) 골절의 진단
● 병력, 증상 징후와 진찰 소견 등을 종합해서 진단한다.
● 외상으로 코가 조금 부었을 때는 육안으로 외상을 입은 코를 보고 쉽게 진단할 수 있다.
● 코에 외상을 입은 후 코피가 심하게 나거나, 코가 막히거나, 코가 비뚤어지거나, 코가 많이 부었거나, 또는 코의 외형이 찌그러질 때는 병원 응급실이나 이비인후과에서 진단 치료 받는다.
● 코 X선 사진 검사로 코뼈 골절을 진단한다.

■ 코 외상과 코뼈(비골) 골절의 치료
● 코 외상의 정도에 따라 치료한다.
● 코뼈(비골) 골절이 없는 단순 코 타박상은 코가 조금 붓고 코피가 조금 날 수 있다.
● 이때 코 얼음물 찬찜질 치료를 한다.
Q&A. 코 외상과 코뼈 골절
● 코피가 나면 코 맨 앞쪽 비익 부분을 한쪽 손의 엄지와 집게손가락 끝부분으로 잠깐 동안 꼭 쥐고 있으면 그 정도의 코피는 멈춘다.
● 코뼈가 골절되었다고 의심하면 이비인후과나 병원 응급실에서 치료를 받는다.
● 코뼈가 골절됐을 때 적절히 치료하지 않으면 코 모양이 이상하게 변형될 수 있어 외관상 문제가 생길 수 있고 건강상 문제가 뒤따라 올 수 있다. 코뼈가 골절됐을 때는 적절한 시기에 적절히 치료받아야 한다.
● 코뼈 골절의 정도에 따라 치료가 다르다.
● 어떤 때는 코뼈가 골절된 후 1주일 정도 기다렸다가 코 외상으로 생긴 부기가 다 내린 후 수술로 치료하기도 한다.
다음은 코 외상과 코뼈 골절에 관한 소아청소년 건강 상담 질의응답의 예입니다.
Q. 아기가 떨어졌는데요

. ● 안녕하세요? 어제 작은(?) 사고가 났습니다. 우리아가는 4개월하고 5일 됐습니다.
● 그런데. 카시트에서 바닥으로 곤두박질을 쳤습니다.
● 애기아빠가… 잘 못 들어서… 한 50cm 높이에서 앞으로 넘어졌는데요. 바닥은 그냥 시멘트 바닥이었고요. 아가가 울어서 얼른 달래 주었지만… 자세히 보니… 겉으로 다른 상처는 없었지만… 한참 뒤에 발견한 거지만. 코 속에..피가 나있었습니다..줄줄 흐르는 정도는 아니었고..약간..비쳐서..묻어있었습니다. 일요일이라 병원도 닫고 해서..우선 집으로 왔는데요.
● 우선 잘 지켜보니…평상시처럼..젖 먹는거나..또..자는 거나..노는 거 보니 똑같길래..그냥..괜찮겠지 했습니다. 그런데… 걱정이 됩니다… 혹시나 코뼈가 부러져 코피가 났나 싶기도 하고… 턱이나… 뭐 이런 데는 괜찮은지… 현재까지는 아무 이상이 없어서 이러고 있는데… 병원에 우선 가야 할까요?
● 부모로서… 항상 아기를 조심해야 하는데 부끄럽습니다만 이렇게 여쭤봅니다.

● 안녕하세요. 질문해 주셔서 감사합니다.
● 자녀의 나이, 성별, 과거 병력, 가족 병력, 진찰 소견, 임상검사 등의 정보를 많이 알수록 답변을 드리는데 도움이 됩니다. 주신 정보를 토대로 해서 답변을 드리겠습니다.
● 다행히 영아들이 높은 곳에서 떨어져도 별 이상이 생기지 않는 때가 많습니다.
● 지금쯤 아기에게 아무 이상이 없을 줄 압니다.
● 코뼈에 골절이 생겼는지 안 생겼는지를 인터넷으로 말씀드릴 수 없습니다.
● 코 뼈는 물렁뼈와 경골로 구성되어 있는데 코뼈가 골절되면 골절된 뼈가 제자리에서 이동되기도 하고 (그림 106 참조)
● 경골과 무릎뼈가 다 골절될 수도 있고,
● 경골만 골절될 수도 있고,
● 경골과 연골 사이가 떨어질 수도 있다.
● 골절이 되고 삐면 코의 모양이 일그러질 수 있습니다.
● 코뼈 골절이 생겼다고 해서 당장 코뼈 골절을 응급치료하는 것보다도 어떤 때는 1주일 정도 관찰 치료를 하다가 코뼈 골절을 치료하기도 합니다.
● 아마도 아기 코뼈의 골절은 생기지 않은 것 같습니다.
● 곧 소아청소년과에서 진단·치료를 받으시기 바랍니다.

■ Causes, Symptoms, and Signs of Nasal Injury and Nasal Fracture
● The nose is the most prominent part of the face, so it can be the first to be injured if the face is hit, such as in an accident.
● Symptoms and signs vary depending on the severity of the nasal injury, its cause, and the presence of other trauma.
● Severe nasal contusions can cause swelling and pain, nosebleeds, and even fractures of the nasal bone.
● The nasal bone consists of the tibia above and cartilage below (see Figure 156). ● Various types of nasal bone fractures can occur, including a minor nasal fracture, a depressed nasal fracture where part of the fractured nasal bone is pushed into the nasal cavity, and a nasal bone fracture where part of the fractured nasal bone is pushed into one side of the nose.

● The nasal cavity is divided into the right and left nasal cavities by the nasal septum.
● If the nasal septum is fractured, one nasal cavity becomes narrower than the other, making the narrower nasal cavity more prone to obstruction.
● This obstruction makes breathing difficult.

■ Diagnosis of Nasal Trauma and Nasal Bone (Nasal Bone) Fracture
● Diagnosis is based on a comprehensive review of medical history, symptoms, signs, and examination findings.
● If the nose is slightly swollen due to trauma, the diagnosis can be easily made by visually examining the injured nose.
● If the nose is severely swollen, stuffy, crooked, swollen, or distorted after a nasal injury, seek diagnosis and treatment at a hospital emergency room or ENT clinic.
● Nasal bone fractures are diagnosed with a nasal X-ray.

■ Treatment of Nasal Trauma and Nasal Bone (Nasal Bone) Fracture
● Treatment is determined by the severity of the nasal trauma.
● Simple nasal contusions without nasal bone (nasal bone) fractures may cause slight nasal swelling and nosebleeds.
● In these cases, cold compresses are applied to the nose.
Q&A. Nasal Trauma and Nasal Bone Fracture
● If you experience a nosebleed, hold the front part of your nose, the alar base, between your thumb and index finger for a short time to stop the bleeding.
● If you suspect a nasal bone fracture, seek treatment at an ENT clinic or a hospital emergency room.
● If a nasal bone fracture is not properly treated, the nose can become deformed, causing cosmetic and health problems. A nasal bone fracture should be treated promptly and appropriately.
● Treatment varies depending on the severity of the nasal bone fracture.
● In some cases, surgical treatment is performed after waiting about a week after the nasal bone fracture and after the swelling from the injury has subsided.
The following is an example of a Q&A session for pediatric and adolescent health counseling regarding nasal trauma and nasal bone fractures.
Q. My baby fell.

. ● Hello? Yesterday(?), there was an accident. Our baby is 4 months and 5 days old.
● But. She fell from the car seat to the floor.
● The baby’s father… didn’t hold her properly… and she fell forward from a height of about 50cm. The floor was just cement. The baby cried, so I quickly comforted her, but… upon closer inspection… there were no other external injuries. I discovered it much later, but… There was blood in her nose. It wasn’t dripping, but it was slightly… visible… and smeared. It was Sunday, so the hospital was closed, so I came home first.
● I observed her closely… and she seemed to be feeding, sleeping, and playing as usual, so I just assumed she was okay. But… I’m worried… I’m wondering if my nosebleed is due to a broken nose… or jaw… or anything like that… There haven’t been any problems so far, so should I take them to the hospital first?
● As a parent… I always have to be careful with my baby, but I’m embarrassed to ask.

● Hello. Thank you for your question.
● The more information I know about your child’s age, gender, past medical history, family medical history, physical examination findings, and clinical tests, the more helpful it is to answer your question. I will answer based on the information you provided.
● Fortunately, many infants fall from high places without any major problems.
● I believe your baby is fine by now.
● I can’t tell you online whether or not there’s a nasal bone fracture.
● The nasal bone is composed of cartilage and tibia. If the nasal bone is fractured, the fractured bone may shift out of place (see Figure 106).
● The tibia and kneecap may both be fractured,
● The tibia alone may be fractured,
● The space between the tibia and the cartilage may be separated.

● If a fracture or sprain occurs, the shape of the nose can become distorted.
● Rather than immediately treating a nasal fracture with emergency treatment, sometimes it’s better to observe the fracture for about a week before treating it.
● Your baby’s nasal bone likely isn’t fractured.

I think it has been more than a century since we needed a parenting information bible in Korean that contains health promotion information that can be easily accessed and read by anyone, anywhere, anytime, to promote health, which is one of the basic necessities of life for Korean men, women, young and old in Korea and around the world.
I have the luxury If I had the time, I would give the entire Parenting Bible to every Korean family around the world.
If I had the time, I would read the entire Parenting Bible aloud to all my beloved Korean parents.
Of course, there may be some shortcomings. I am confident that if you raise your children while referring to other parenting bibles, they will grow up happily.
