1. 소아 건강검진 진료 기록지(외래) Ⅰ
소아가 어떤 병으로 소아청소년과에 처음 치료받으러 왔을 때나 건강한 소아가 정기 건강검진을 받으러 소아청소년과에 처음으로 왔을 때
그 소아의 어머니의 임신과 출산의 병력,
그 소아의 과거와 현재 병력,
가족 병력,
부모와 친 형제자매들의 가족 병력,
소아 예방접종에 관한 정보,
성장발육 이정표,
음식물 먹는 습관,
부모의 사회생활에 관련 사항 등의 정보를 기록할 수 있게 만든 소아 외래 건강검진 진료 기록지(I)가 있고

2. 소아 건강검진 진료 기록지(외래) Ⅱ

맨 처음 보는 소아 환자나 건강한 소아를 건강검진을 한 바로 후 진찰소견, 진단, 문제, 치료, 임상검사, 앞으로 할 치료 계획 등을 기록할 수 있게 만든 소아 건강검진 외래 진료 기록지다.

3. 소아 건강검진 진료 기록지(외래) III
구 소아 환아를 추적 재검진할 때, 또 소아 건강검진 의료 기록지 외래 Ⅰ은 추적 정기 건강검진을 할 때 이용할 수 있는 소아 건강검진 외래 진료 기록지이다.
물론 필요에 따라 소아 건강검진 외래 진료 기록지 Ⅰ을 처음 방문한 이후 또 재차 방문했을 때마다 소아 건강검진 외래 진료 기록지 Ⅰ을 참고하고 최신 정보를 다시 가감한다.
여기서 3종류의 소아 건강검진 외래 진료 기록지 Ⅰ, Ⅱ, Ⅲ를 소개한다.
이 건강검진 외래 진료 기록지를 작성한 후 소아과학에 많은 발전과 변화가 생겼다.

1. Pediatric Health Checkup Record (Outpatient) I

When a child first visits the Pediatrics Department for treatment for a medical condition or a healthy child first visits the Department for a regular health checkup,
the Pediatric Outpatient Health Checkup Record (I) is available to record information such as the child’s mother’s pregnancy and childbirth history,

the child’s past and present medical history,

family medical history of the parents and siblings,

information on childhood immunizations,

growth milestones,

eating habits,

and information related to the parents’ social life.

2. Pediatric Health Checkup Record (Outpatient) II

The Pediatric Outpatient Health Checkup Record allows you to record the examination findings, diagnosis, problems, treatments, clinical tests, and future treatment plans immediately after a health checkup on a pediatric patient or a healthy child. This is a record.

3. Pediatric Health Checkup Outpatient Clinic Record III

This Pediatric Health Checkup Outpatient Clinic Record can be used for follow-up re-examinations of older children, and Pediatric Health Checkup Outpatient Clinic Record I can be used for follow-up regular checkups.

Of course, if necessary, Pediatric Health Checkup Outpatient Clinic Record I can be referenced at each subsequent visit, adding or subtracting the latest information.

Here, we introduce three types of Pediatric Health Checkup Outpatient Clinic Record I, II, and III.
Since the creation of this outpatient clinic record, pediatrics has undergone many advancements and changes.

In particular, various new vaccines have been developed, and vaccination methods are constantly changing. Depending on the region and country where the child lives, it is appropriate to modify the pediatric health checkup outpatient clinic record accordingly, based on the recommended vaccine types and the country’s pediatric health promotion laws.
