Medications for the Treatment of Drug Overdose or Chemical Poisoning

Medicines that can be kept at home for emergency use when needed include Ipecac syrup and activated charcoal.
1. Ipecac syrup

● Poisoning can occur when drugs, poisons, or chemicals are accidentally ingested orally or when ingested in a suicide attempt.
● If you suspect or believe you may have been poisoned by a chemical or drug after oral ingestion, seek emergency medical attention from a general hospital poison center, hospital emergency room, poison center (1-800-222-1222 in the US), or your primary care physician. Follow their instructions and begin appropriate treatment on-site.
● If necessary, induce vomiting or seek assistance from an ambulance or other medical professional. Use the fastest and most convenient transportation to the emergency room for follow-up emergency care.
● Until 2003, the American Academy of Pediatrics recommended that if there was a risk of poisoning by oral ingestion of a poison, oral ipecac syrup could be used to induce vomiting of the medication or poison in the stomach. However, since 2003, the American Academy of Pediatrics has recommended that ipecac syrup not be routinely stored at home. The reasons are as follows:
1) Even when pediatric poisoning is treated at home with ipecac syrup, the overall outcome of treatment with ipecac syrup is no better than when no ipecac syrup is used.
2) Even with ipecac syrup treatment, the patient does not vomit out all the ingested poison.
3) Parents indiscriminately administer ipecac syrup emergency treatment to children who have ingested non-toxic substances.
4) Parents arbitrarily treat poisoning with ipecac syrup without consulting a doctor.
5) In cases where parents have already treated poisoning at home with ipecac syrup, continued vomiting sometimes prevents the oral medications required for ipecac treatment.
For these reasons, we no longer recommend storing ipecac syrup at home to treat poisoning caused by oral ingestion.
However, it is recommended to check with your pediatrician to determine whether home storage is permitted, depending on the medical situation in your country. ● Sometimes, in the emergency room, ipecac syrup is administered orally as needed. A nasopharyngeal catheter (tube) is inserted into the stomach, and ipecac syrup is infused into the stomach through the tube. This induces vomiting of gastric toxicants or poisons within 10-15 minutes, preventing and treating poisoning.
● The single therapeutic dose of ipecac syrup varies depending on age. It should be administered according to the pharmaceutical company’s directions.
● Vomiting usually occurs 10-15 minutes after administering the appropriate dose orally.
● Ipecac comes in two forms: ipecac syrup and ipecac concentrate.
● Depending on the prescription, ipecac syrup can be kept at home and used as needed after consulting a doctor.
● Using ipecac concentrate can lead to ipecac concentrate poisoning.
● Do not store Ipecac concentrate at home without a doctor’s direction, nor use it to treat drug poisoning, etc.

● If you suspect poisoning after accidentally ingesting drugs or poisons orally or for other reasons, you can treat the situation by administering 15-30 cc of Ipecac syrup stored at home, as directed by a doctor.
● Ipecac syrup can also be used under a doctor’s direction for other reasons when urgent vomiting of stomach contents or ingested material is necessary.
● In general, do not induce vomiting with Ipecac syrup if the patient accidentally ingests chemicals such as gasoline, kerosene, or kerosene.
● Ingestion of small amounts of kerosene or mineral oil can be treated by inducing vomiting with Ipecac syrup. This should also be treated under a doctor’s instructions and judgment.
● Do not induce vomiting with ipecac syrup if the patient accidentally drinks or eats strong acids or alkalis, such as lye or sulfuric acid.
● Do not induce vomiting with ipecac syrup if the patient is vomiting due to an illness, has lost the gag reflex, is convulsing, or is in a coma.
● Ideally, before treating poisoning with ipecac syrup orally, consult a hospital emergency room, your primary care physician, or a poison control center to determine if ipecac syrup is appropriate and begin treatment accordingly.
● In cases of accidental ingestion of poisons or overdoses, more treatment beyond inducing vomiting of the ingested poison is often necessary, and further poison control may be necessary.
● After inducing vomiting with Ipecac syrup as directed by a doctor, take the patient to the hospital emergency room or poison treatment center by ambulance or other appropriate means of transportation as directed by the emergency room of a general hospital, regular doctor, or poison treatment center.
1) Ipecac Syrup and Poisoning Treatment
● In 1940, approximately 500 children in the United States died from poisoning, and in 1997, 25 died.
If a child was at risk of or had already been poisoned by ingesting drugs or poisons due to an accidental safety incident, the child was treated with Ipecac syrup, inducing vomiting of the poison, as directed by a doctor, emergency room, or poison control center.
Therefore, it was recommended that every household with children keep a 30cc bottle of Ipecac syrup. Recently, the following treatment method has been recommended: The American Academy of Pediatrics recently released a policy on emergency poisoning, recommending that healthcare providers caring for infants, toddlers, and school-aged children routinely educate parents about the risks of ingestion-related poisoning in children.
The American Academy of Pediatrics recommends that ipecac syrup should not be routinely used to treat pediatric poisoning, as previously recommended.
The American Academy of Pediatrics recommends that ipecac syrup be used only under the direction of a physician or other healthcare provider.
The American Academy of Pediatrics recommends that parents routinely store activated charcoal at home without a physician’s direction.
If poisoning is suspected, parents should call the Poison Control Center at 1-800-222-1222 for advice and treatment.
There have also been cases of ipecac syrup inducing vomiting, resulting in “Munchausen Syndrome by Proxy.”
● Due to cases of improper poisoning treatment and cases of obesity being treated by inducing vomiting with Ipecac syrup, it has been announced that it is no longer recommended to store Ipecac syrup at home.

● However, people living outside the United States cannot necessarily treat poisoning according to this recommendation and should treat poisoning according to their country’s medical policies.
2. Activated Charcoal Powder
● Activated charcoal powder is a type of charcoal powder.
● If you ingest an overdose of medication or poison, whether due to a safety accident or other reasons, taking activated charcoal can absorb bacteria, drugs, or poisons in the gastrointestinal tract.
● When activated charcoal comes into contact with drugs, poisons, or bacteria in the gastrointestinal tract, the drug, poison, or bacteria can be absorbed by the activated charcoal.
● Activated charcoal comes in powder and suspension forms.
● The Korean Academy of Pediatrics does not, in principle, recommend storing powdered or suspended activated charcoal at home.
● Depending on your living conditions and circumstances, consult your doctor before storing activated charcoal at home to avoid accidental ingestion of poisons or chemicals.

. ● It can be used when suspected of substance or drug poisoning or when providing first aid at home under the direction of a doctor.
● Activated charcoal can be taken orally or injected into the gastrointestinal tract through a nasogastric tube before poisons or drugs are absorbed into the bloodstream. This prevents poisoning and can alleviate the symptoms of existing poisoning.
● When a patient is unable to ingest activated charcoal orally, a nasogastric tube can be inserted through the nostrils, nasal cavity, pharynx, esophagus, and stomach. This tube can then be used to deliver activated charcoal into the gastrointestinal tract to prevent and treat poisoning or drug intoxication.
● The tube can also be inserted through the mouth into the pharynx, esophagus, and stomach.
● Activated charcoal suspension is sold in 15 g or 50 g bottles.
● It is recommended to purchase one of these three sizes of activated charcoal suspension and keep it at home for use as needed, as directed by a physician.
● Considering the possibility that a child living in a remote mountain or rural area with limited medical facilities might suffer a poisoning accident and require significant time to reach a hospital for treatment, keep Ipecac syrup and activated charcoal powder at home. If a poisoning accident occurs or is suspected, these medications can be used at home under the direction of a pediatrician or hospital emergency room.
● ChelcoAidz and Actidose-Aqua are brand names for activated charcoal powder suspension.

3. Other Drugs for Poisoning Treatment
● When ingesting chemicals that can irritate the gastrointestinal tract or are corrosive, emergency treatment can be provided by giving plain water or milk to dilute the substance in the gastrointestinal tract.
● However, this treatment should also be administered as an emergency, following a doctor’s instructions.
● When there is a risk of poisoning due to a drug requiring delayed absorption, treatment can be with a drug that induces sodium sulfate.
● This treatment should also be administered following a doctor’s instructions.

I think it has been more than a century since we needed a parenting information bible in Korean that contains health promotion information that can be easily accessed and read by anyone, anywhere, anytime, to promote health, which is one of the basic necessities of life for Korean men, women, and children in Korea and around the world.
