● Jaundice can occur in newborns due to ABO blood type incompatibility between the mother’s blood type and the fetus’s.
● Neonatal jaundice caused by ABO blood type incompatibility is called ABO incompatibility jaundice.
● If the ABO blood types of the mother and the fetus are different, the newborn can develop ABO incompatibility jaundice.
● When a newborn born to a mother with blood type O has blood type A or B, the newborn is more likely to develop ABO incompatibility (see Table 143), which increases the likelihood of developing jaundice due to ABO incompatibility.
● Pregnant women with blood type O normally have A and B antibodies in their blood.
● These antibodies can cause hemolysis of the red blood cells of newborns with blood type A or B.
● If ABO blood type antibodies in the pregnant woman’s blood enter the fetus’s bloodstream, the fetus’s A or B red blood cells can be destroyed.
● This can lead to neonatal jaundice and anemia due to ABO blood type incompatibility in the fetus.
● The ABO blood types of the pregnant woman and the newborn are often different.
● For example, the pregnant woman’s blood type may be A, but the newborn’s blood type may be B. Therefore, the likelihood of a newborn developing neonatal jaundice due to ABO blood type incompatibility is theoretically much higher than the likelihood of neonatal jaundice due to Rh factor incompatibility.
● However, during pregnancy, A or B blood type antibodies in the pregnant woman’s blood, which could cause hemolysis of the fetus’s red blood cells due to ABO incompatibility, do not easily cross the placenta and enter the fetus’s bloodstream.
● Therefore, the incidence of neonatal jaundice due to ABO incompatibility is much rarer than expected.
● Approximately 15% of newborns develop jaundice, a condition predicted by ABO blood type incompatibility.
● In reality, neonatal jaundice caused by ABO blood type incompatibility rarely causes serious health problems.
■ Signs and Symptoms of Neonatal Jaundice Caused by ABO Blood Type Incompatibility
● Signs and symptoms vary greatly depending on the cause and severity of the jaundice.
● In mild cases of neonatal jaundice caused by ABO blood type incompatibility, the blood concentration of indirect bilirubin may be slightly higher than normal, with no jaundice at all.
● The severity and signs of neonatal jaundice caused by ABO blood type incompatibility are typically much milder than those caused by Rh incompatibility.
● In mild cases, the newborn’s skin and whites of the eyes may be slightly yellow, and anemia may be mild.
● When ABO blood type incompatibility is severe, the symptoms and signs of neonatal jaundice caused by Rh incompatibility can be very similar to those of Rh incompatibility.
● Unlike Rh incompatibility, neonatal jaundice caused by ABO blood type incompatibility is more common in first-time newborns.
■ Diagnosis of neonatal jaundice caused by ABO blood type incompatibility
● Diagnosis is based on a combination of symptoms and signs, including yellowing of the newborn’s skin and whites of the eyes, physical examination findings, and the results of maternal and newborn Rh factor tests, ABO blood type tests, Coombs tests, and indirect bilirubin blood tests.
■ Treatment of neonatal jaundice caused by ABO blood type incompatibility
● Mild jaundice caused by ABO blood type incompatibility requires no treatment.
● Severe jaundice requires hospitalization in a neonatal intensive care unit. ● When a newborn’s blood level of indirect bilirubin continues to rise abnormally, frequent blood tests may be necessary to monitor the level.
● Indirect bilirubin levels should be monitored at regular intervals until the level stops increasing and approaches normal levels.
● Bilirubin phototherapy may be used long before indirect bilirubin levels reach a health-threatening level.
● Dehydration should also be treated appropriately.
● Exchange transfusion therapy may be used in cases of severe jaundice and anemia. However, exchange transfusion therapy is rarely necessary.
● When treating neonatal jaundice with bilirubin phototherapy, the cause must be identified first.
● When treating jaundice with bilirubin phototherapy, the eyes are covered with an eye patch to prevent light damage to the retina.

17. I believe that it has been more than a century since we needed a parenting information bible in Korean that anyone can easily access and read anytime, anywhere to promote health, which is one of the basic necessities of life for Koreans of all ages in Korea and around the world.
