When a child or adolescent first comes to my pediatric clinic for treatment, I ask the child’s parents or guardians to fill out everything on this record sheet, including “Why do you bring your child here today?”. After they complete it, I usually ask any remaining questions and listen to their answers, then I meticulously examine the child from head to toe, recording my findings. This record sheet has received much praise in the United States. It would be good if it were translated into other languages.
