● 그 소아의 어머니의 임신과 출산의 병력,
● 그 소아의 과거와 현재 병력,
● 가족 병력,
● 부모와 친 형제자매들의 가족 병력,
● 소아 예방접종에 관한 정보,
● 성장발육 이정표,
● 음식물 먹는 습관,
● 부모의 사회생활에 관련 사항 등의 정보를 기록할 수 있게 만든 소아 건강 검진 의료 기록지(I)가 있고

2. 소아 건강 검진 의료 기록지(외래) Ⅱ
● 맨 처음 보는 소아 환자나 건강한 소아를 건강검진을 한 바로 후 진찰소견, 진단, 문제, 치료, 임상검사, 앞으로 할 치료계획 등을 기록할 수 있게 만든 소아 건강검진 의료 기록지이다.

● 구 소아 환아를 추적 재검진할 때, 또 소아 건강 검진 의료 기록지 (외래) Ⅰ은 추적 정기 건강검진을 할 때 이용할 수 있는 소아 건강검진 의료 기록지이다.
● 물론 필요에 따라 소아 건강검진 의료 기록지Ⅰ을 처음 방문한 이후 또 재차 방문했을 때마다 소아 건강검진 의료 기록지 Ⅰ을 참고하고 최신 정보를 다시 가감한다.

1. Pediatric Health Checkup Medical Record (Outpatient) I
● When a child first visits the pediatrics department for treatment for a medical condition or a healthy child first visits the pediatrics department for a regular checkup,
● the child’s mother’s pregnancy and childbirth history,
● the child’s past and present medical history,
● family medical history,
● the family medical history of parents and siblings,
● information on childhood immunizations,
● growth and development milestones,
● eating habits,
● information related to the parents’ social life,

2. Pediatric Health Checkup Medical Record (Outpatient) II
● When a child is first seen or a healthy child is immediately examined, the medical findings, diagnosis, problems, treatments, clinical tests, and future treatment plans can be recorded. This is a pediatric health checkup medical record.
