Acute Diarrhea
■ Overview and Causes of Acute Diarrhea
Diarrhea lasting two weeks or less is called acute diarrhea, while diarrhea lasting more than two weeks is called chronic diarrhea.
The most common causes of acute diarrhea are viral or bacterial infections of the gastrointestinal tract.
Rotavirus, Norwalk virus, enterovirus (enterovirus), or other viral infections of the gastrointestinal tract can cause acute diarrhea. Typical symptoms of acute diarrhea include watery diarrhea, vomiting, mouth odor, and dehydration.
In contrast, bacterial infections such as Salmonella (p.00), Shigella (p.00), or pathogenic Escherichia coli (p.00) that cause gastroenteritis and result in acute diarrhea include vomiting, mouth odor, bloody stool, mucus in the stool, abdominal pain, and dehydration. Food poisoning caused by Staphylococcus aureus bacteria typically results in watery diarrhea.
E. coli bacterial gastrointestinal infections can also cause diarrhea, often with only watery stools.
Acute diarrhea can also be caused by giardiasis (p. 00).
■ Diagnosis and Treatment of Acute Diarrhea
Diagnosis is made based on a comprehensive review of medical history, symptoms, and examination. If necessary, a stool microscopic examination or stool culture is performed.
Treatment is determined by the cause and, if necessary, symptomatic treatment is provided. (See p. 00 Food Poisoning.)
The following is an example of a Q&A from the online Pediatric and Adolescent Health Counseling service regarding diarrhea in babies over a month old.
Q&A. My baby has had diarrhea for over a month.
Hello. I visit often and get a lot of helpful information.
My daughter is currently 11 months old, and it’s been about a month and a half since she started having diarrhea. For the first week, I was treated at the hospital. They just said it was enteritis. After that, when I fed him hops formula (for diarrhea), he didn’t have diarrhea, but even if I mixed just 1/3 of it with regular formula, he started having diarrhea. He had no problems drinking milk until he was 9 months old, but now he looks like he has a milk allergy. I’m worried that if I continue to feed him only hops formula, he might have nutritional deficiencies. He doesn’t have any symptoms like tummy groans or sagging, so I don’t think he has enteritis, but I don’t know how to provide him with nutrition. He was fine when I fed him small amounts of rice before, but would it be okay to make rice porridge and feed him? Until now, I’ve stopped eating all foods except water, sports drinks, and hops formula (for diarrhea).
A. Hello, Mi-ae. Thank you for your helpful question. The more information we have about your child’s age, gender, past medical history, family medical history, physical findings, and clinical tests, the more helpful it is for us to answer your question. We will provide you with an answer based on the information you have provided.
Acute diarrhea is defined as diarrhea lasting for two weeks or less, and chronic diarrhea is defined as diarrhea lasting more than two weeks.
Based on these definitions, it appears your baby has chronic diarrhea.
The most common cause of acute diarrhea in infants is viral gastroenteritis, followed by bacterial gastroenteritis. Acute diarrhea caused by viral gastroenteritis is usually watery and accompanied by vomiting. Acute diarrhea caused by bacterial gastroenteritis is usually bloody or loose stools and accompanied by abdominal pain.
Chronic diarrhea can be caused by infections with Giardia falciparum, such as giardiasis. Aside from gastrointestinal infections caused by pathogenic agents, the main causes of chronic diarrhea in infants due to gastrointestinal pathogenic agents are milk protein allergy, soy protein allergy, and lactose intolerance caused by the lactose in milk.
The main symptoms of milk protein or soy protein allergic colitis include poor appetite, vomiting, diarrhea, and bloody or loose stools.
In these cases, you should stop feeding your infant formula containing soy protein or milk protein and instead provide a special formula made with hydrolyzed protein.
There are two types of special formulas made with hydrolyzed protein, such as Nutramigen, or hypoallergenic formulas: those made with partially hydrolyzed protein and those made with extensively hydrolyzed protein. These types of formulas are called hypoallergenic formulas. Even with this special formula, protein allergies cannot be 100% prevented.
In the United States, you can choose between Nutramizine or Progest Meal, which are hypoallergenic formulas.
In Korea, you can choose a hypoallergenic formula made with similar ingredients to Nutramizine.

Lactose is a type of carbohydrate and does not cause allergies. However, lactose intolerance can occur due to a congenital or acquired deficiency of the lactase enzyme, which can lead to diarrhea in infants and young children.
The following treatment options are recommended:
Get a diagnosis at your local pediatrician. Determine the cause or disease of the diarrhea and perform appropriate stool, urine, and blood tests.
Perform a stool test to determine if there are any gastrointestinal bacterial infections, gastrointestinal bleeding, parasitic infections, or Giardia infections. Avoid feeding your baby any foods containing milk protein, formula, or soy protein for about two weeks, and continue to feed only hop-based formula as directed by your doctor.
Only feed your baby rice-based grain baby food and any other baby food you have previously fed that you are certain does not cause any allergies or intolerances.
When introducing baby food, try to feed only one type of food for the first time. If you must add a different type of food to the previous one, wait at least a week before introducing the new type of food.
If there are no problems with the new food, try mixing the first and second types of food.
Check for weight gain due to diarrhea. Ionic drinks are not very effective in treating chronic diarrhea.

I think it’s been over a century since we needed a parenting information bible in Korean that contains health promotion information that anyone can easily access and read, anywhere, anytime, to promote health, which is one of the basic necessities of life for Koreans of all ages in Korea and around the world.
If you have the time, please visit the Korean website of Dr. Leesangwon. I would like to give the entire Parenting Bible to families.
If I had time, I would love to read the entire Parenting Bible aloud to my beloved Korean parents.
Of course, there may be some shortcomings. I am confident that if you refer to other parenting bibles while raising your children, your children will undoubtedly grow up happy.
