● Dry eye syndrome is a condition in which the eyes do not produce enough tears to maintain optimal eye health, resulting in dryness and discomfort.
● In rare cases, tears may not be produced at all.
● This condition is called alacrimation.
● Normal tears are composed of water, fat, and mucus.
● While these three components are balanced, certain stimuli can cause the tears to contain too much water. ● Tears are slowly and continuously produced and secreted from the lacrimal glands, depending on the degree of eye dryness, to moisturize the eye.
● Fat and mucus, like water, are components of tears. Among the tear components, fat and mucus act as ocular lubricants.
● Tears smooth eye movement and help wash away foreign substances.
● Emotional changes can lead to increased tear production, such as crying, and sudden increases in tear production can occur when wiping away foreign substances.
● The tear film layer is constantly formed on the surface of the eye.
● The tear film layer consists of three layers: a lipid layer, a water layer, and a mucus layer.
● The lipid layer is formed from lipids secreted by the meibomian glands.
● The tear film layer plays a crucial role in maintaining normal vision.
● The water secreted by the lacrimal glands cleanses the eye and helps wash away foreign substances.
● The mucus in the tear film layer is produced in the inner layer of the conjunctiva. ● Mucus forms the mucous membrane layer and helps tears adhere well to the eye.
■ Causes of Dry Eye
● As you age, tear production can naturally decrease.
● Birth control pills, antidepressants, antihistamines, decongestants, anti-anxiety medications, diuretics, sleeping pills, painkillers, and blood pressure medications can cause dry eye.
● Hormonal changes during menopause, pregnancy, and breastfeeding can also cause dry eye.
● Allergic reactions can cause dry eye.
● Autoimmune diseases such as Sjogren’s syndrome, rheumatoid arthritis, and myasthenia gravis, as well as Bell’s palsy and thyroid disease, can cause dry eye.
● Anhydraemia can occur in people with familial dysautonomia (Lilly-Day syndrome), which prevents tear production.
■ Symptoms of Dry Eye
① Eyes may sting and burn.
② Sticky mucus glands may be visible.
③ Eyes may feel like they are being scratched with a fingernail.
④ Sensitivity to light.
⑤ Difficulty wearing contact lenses.
⑥ Increased tear production.
⑦ Eyes may become sensitive when exposed to smoke or wind.

■ Diagnosis of Dry Eye
① Diagnosis is based on symptoms, past medical history, family history, eye examination, physical examination, and blood tests.
② Spot dry spots on the cornea by placing a dyed strip on the eye.
③ Analyze tear components.
④ Perform a Schirmer test.
⑤ Perform a Rose Bengal test.
■ Treatment of Dry Eye
① Treat by removing the cause.
② Avoid and eliminate factors that can worsen dry eye, such as wind, smoke, chemical vapors, dry weather, hair dryers, and air conditioners. ③ Use a humidifier indoors.
④ Stop wearing contact lenses.
⑤ Avoid using a computer for long periods of time.
⑥ Wear safety glasses or goggles.
⑦ Use artificial tears or tear ointment.
⑧ Treatment can be used to preserve tears by blocking the openings of the nasolacrimal ducts that drain tears into the nose.
⑨ Topical cyclosporine eye drops, a tear production-stimulating agent, can be used.
⑩ Increase indoor humidity with a humidifier or a bowl of water.
⑪ Control the indoor temperature, avoid drafts, smoking, hair dryers, etc.
