What is a peanut allergy?

Peanut allergy involves an immune reaction to peanuts that may cause skin, gastrointestinal, respiratory, and cardiovascular symptoms, and can cause severe allergic reactions.

 

Peanut allergy is a leading cause of food allergy worldwide and affects about 2% of people in the US. Most patients develop peanut allergy in early childhood, and about 10% to 30% outgrow it.1

 

Symptoms of peanut allergy usually develop within 1 to 2 hours of eating peanuts (often within minutes) and may occur after ingestion of even trace amounts. Allergic reactions may affect the skin (swelling, hives), gastrointestinal tract (vomiting, diarrhea), respiratory system (cough, wheeze), and cardiovascular system (low blood pressure or shock). Anaphylaxis is a life-threatening allergic reaction, but death is low (approximately 1 in 10 million).

 

How Is Peanut Allergy Diagnosed and Treated?

 

Peanut allergy is diagnosed based on a history of allergic symptoms within 2 hours of eating peanuts, and either a positive skin prick test (a skin reaction that develops within 10 to 20 minutes after an allergist pricks a small amount of peanut protein into the surface of the skin) or a blood test that detects specific immunoglobulin E (IgE) antibodies to peanut and specific peanut proteins.

 

First-line treatment for a severe allergic reaction is epinephrine (given via injection or nasal spray), which significantly reduces the risk of death from anaphylaxis. Repeat doses of epinephrine should be given every 5 to 15 minutes for patients whose anaphylactic symptoms have not resolved. Additional medications may include antihistamines, intravenous epinephrine, and drugs to increase blood pressure. Patients treated in the emergency department also may receive intravenous fluids.

 

Oral immunotherapy is an approved therapy by the US Food and Drug Administration (FDA) for peanut allergy. This therapy involves ingestion of gradually increasing amounts of peanut protein every 2 to 4 weeks to reach a dose equivalent to about 1 to 2 small peanuts, with daily peanut ingestion at home between visits. Oral immunotherapy successfully desensitizes about 80% of individuals with peanut allergy, allowing them to tolerate small amounts of peanut, but requires ongoing regular peanut ingestion and it is associated with a risk of anaphylaxis of about 14%.

 

In February 2024, the FDA approved a biologic medication (omalizumab) for peanut and other food allergies for people aged 1 year or older with qualifying levels of IgE. Omalizumab is injected below the skin every 2 or 4 weeks with dosing based on the patient’s weight and baseline IgE level. The first 3 doses are given in an outpatient clinic to monitor for anaphylaxis, after which the medication can be taken at home. About two-thirds of patients with peanut allergy taking omalizumab are desensitized to peanuts.

 

When Should Infants Start Eating Peanut-Containing Foods?

 

A 2017 guideline from the National Institute of Allergy and Infectious Diseases issued a recommendation that peanut-containing foods should be introduced into the diet during infancy. This recommendation was based on a study reporting substantially lower rates of peanut allergy among infants with eczema and/or egg allergy who started eating peanut-containing foods at aged 4-11 months vs aged 5 years. Since the 2017 recommendation of earlier introduction of peanut-containing foods, a 2025 study reported a significant decrease in the diagnosis of peanut allergy in infants. Peanuts should not be given to infants or any young child before the eruption of molar teeth because of the risk of aspiration of nuts and fragments.

땅콩 알레르기는 땅콩에 대한 면역 반응을 수반하며, 피부, 위장관, 호흡기 및 심혈관계 증상을 유발할 수 있고 심각한 알레르기 반응으로 이어질 수도 있습니다.

땅콩 알레르기는 전 세계적으로 식품 알레르기의 주요 원인 중 하나이며, 미국 인구의 약 2%가 앓고 있습니다. 대부분의 환자는 유아기 또는 어린 시절에 땅콩 알레르기가 발병하며, 약 10%에서 30% 정도는 성장하면서 자연스럽게 알레르기가 사라지기도 합니다.1

땅콩 알레르기 증상은 보통 땅콩 섭취 후 1~2시간 이내(종종 수 분 이내)에 나타나며, 아주 미량의 땅콩을 섭취했을 때도 발생할 수 있습니다. 알레르기 반응은 피부(부기, 두드러기), 위장관(구토, 설사), 호흡기계(기침, 천명음), 심혈관계(저혈압 또는 쇼크) 등 신체 여러 부위에 영향을 미칠 수 있습니다. 아나필락시스(Anaphylaxis)는 생명을 위협할 수 있는 심각한 알레르기 반응이지만, 이로 인한 사망률은 매우 낮은 편입니다(약 1,000만 명 중 1명꼴).

 

땅콩 알레르기는 어떻게 진단하고 치료하는가?

땅콩 알레르기는 땅콩 섭취 후 2시간 이내에 알레르기 증상이 나타났던 병력을 바탕으로 진단합니다. 여기에 더해, 양성 반응을 보인 피부 단자 검사(알레르기 전문의가 피부 표면에 소량의 땅콩 단백질을 주입한 후 10~20분 이내에 피부 반응이 나타나는 검사)나, 땅콩 및 특정 땅콩 단백질에 대한 특이 면역글로불린 E(IgE) 항체를 검출하는 혈액 검사 결과를 종합하여 확진하게 됩니다.

심각한 알레르기 반응에 대한 1차 치료제는 에피네프린(주사 또는 비강 스프레이 형태로 투여)이며, 이는 아나필락시스로 인한 사망 위험을 현저히 낮춰줍니다. 아나필락시스 증상이 호전되지 않는 환자의 경우, 5~15분 간격으로 에피네프린을 반복 투여해야 합니다. 추가적으로 투여할 수 있는 약물로는 항히스타민제, 정맥 주사용 에피네프린, 혈압 상승제 등이 있습니다. 응급실에서 치료받는 환자는 정맥 수액을 투여받을 수도 있습니다.

경구 면역 요법은 미국 식품의약국(FDA)에서 승인한 땅콩 알레르기 치료법입니다. 이 치료법은 2~4주 간격으로 땅콩 단백질 섭취량을 점진적으로 늘려 작은 땅콩 1~2알에 해당하는 용량에 도달하도록 하는 것입니다. 병원 방문 사이에는 집에서 매일 땅콩을 섭취해야 합니다. 경구 면역 요법은 땅콩 알레르기 환자의 약 80%에서 성공적으로 탈감작을 유도하여 소량의 땅콩을 섭취할 수 있게 하지만, 지속적인 규칙적인 땅콩 섭취가 필요하며 약 14%의 아나필락시스 위험과 관련이 있습니다.

2024년 2월, FDA는 IgE 수치가 기준치를 충족하는 1세 이상 환자의 땅콩 및 기타 식품 알레르기 치료를 위해 생물학적 제제(오말리주맙)를 승인했습니다. 오말리주맙은 환자의 체중과 기저 IgE 수치에 따라 용량을 조절하여 2주 또는 4주 간격으로 피하 주사합니다. 처음 3회 투여는 아나필락시스 반응 모니터링을 위해 외래 진료실에서 시행하며, 이후에는 자택에서 복용할 수 있습니다. 오말리주맙을 복용하는 땅콩 알레르기 환자의 약 2/3에서 땅콩에 대한 탈감작이 나타납니다.

영유아는 언제부터 땅콩 함유 식품을 섭취해야 할까요?

미국 국립알레르기감염질환연구소(NIAID)는 2017년 가이드라인에서 영유아기에 땅콩 함유 식품을 식단에 도입할 것을 권고했습니다. 이 권고는 습진 및/또는 계란 알레르기가 있는 영유아를 대상으로 생후 4~11개월에 땅콩 함유 식품 섭취를 시작한 경우 5세에 시작한 경우보다 땅콩 알레르기 발생률이 현저히 낮다는 연구 결과를 바탕으로 합니다. 2017년 땅콩 함유 식품 조기 도입 권고 이후, 2025년 연구에서는 영유아의 땅콩 알레르기 진단 건수가 유의미하게 감소한 것으로 보고되었습니다. 땅콩은 어금니가 나기 전의 영유아에게 주어서는 안 됩니다. 땅콩 알갱이나 조각이 흡인될 위험이 있기 때문입니다.
