미국인의 뇌졸중 역학 

미국 질병통제예방센터(CDC)에 의하면

미국인 20명 중 1명이 뇌졸중으로 사망한다.

·미국에서  매 40초 마디 1명이 뇌졸중에 걸린다.

· 매 4분마다 한 명이 뇌졸중으로 죽는다.

· 매년 마다 미국사람 750,000~795,000 명 이상이  뇌졸중에  걸린다.  

·795,000 명 중 610.000명은 처음 또는 재차로 뇌졸중에 걸렸다.

· 뇌졸중 생존자의 ¼ 즉 185,000명(25%)은 과거에 뇌졸중에 걸린 병력이 있다.

·전체 뇌졸중 환자 중 87%는 허혈성 뇌졸중에 걸리고, 나머지 13%는 출혈성 뇌졸중에 걸린다.

· 과거 한번 또는 그 이상 일과성 허혈성 뇌졸중(TIA) 에 걸린 병력이 있으면 TIA에 걸린 경험이 없는 동성, 동연령 대조군에 있는 사람들에 비해 뇌졸중에 더 잘 걸일 수 있다.

· 담배를 피우지 않는 사람에 비해 담배를 피우는 사람들은 허혈성 뇌졸중에 걸릴 위험이 2배 이상 다.

· 미국 뇌졸중 생존자들의 건강 관리하는하는데 드는 비용와 뇌졸중 치료비, 약값, 뇌졸중으로 인해 일을 하지 못해 벌지 못 하는 수입 등을 총 합치면 년간 34억불이 든다고 추정한다.

· 뇌졸중 생존자들은 장기적으로 심각한 불구자로 살아가게 되는 경우가 보통이다.

·급성기 응급 뇌졸중 치료를 받고  회복된 65세 이상 사람들의  50%는 뇌졸중으로 인한 신체적 운동 불구자가 된다.

·첯 뇌졸중에 걸린후 30일 이내 3%-10%, 1년 이내에 55-14%. 5년 이내에 25-40%가 재발한다. 그리고 첫 발병 후 5년 내 여성은 42%, 남성은 24%가 재발한다.

·  미국에서는 뇌졸중 5번째 사망 원인이 된다. 그러나 인종에 따라 그 통계는 조금  다르다. 미국에서 흑인은 처음 뇌졸중에 걸리는 확률이 백인에 비해 배가되고, 뇌졸중으로 죽는 확률도 백인에 비해 상당히 더 높다.

· 뇌졸중의 발병 위험성은 나이에 따라 달라진다. 뇌졸중은 어느 연령층 사람들에게도 발병할 수 있으나 나이를 더 들면서 더 잘 생긴다.

· 미국에서 2009년도 연구에 의하면  뇌졸중으로 병원에 입원한 환자들 중 34%는  65세 이하 사람들이었다.

· 2018년 8월 말, 한국 치매 안심센터에 등록한 노인들의 수는 23만 8,077명이고 치매 노인들의 수는 72만 4,734명이었다.

미 내숀날 뇌졸중 협회(American National Stroke Association)에

의하면 다음과 같은 뇌졸중 생존자들은 회복된다.

1. 10% 뇌졸중 생존자들은 거의 완전히 회복되고

2. 25% 뇌졸중 생존자들은 일부 장애를 남기고 회복되고

3. 40% 뇌졸중 생존자들은 중등도 내지 중증도 장애를 동반하면서 특수

재활 치료를 받고 있다. 직장 생활을 할 때나 일상 생활을 할때 매일

필요한

기능장애를 계속 가지고 산다고 한다.

4. 10% 뇌졸중 생존자들은 너싱 홈이나 그외 다른 요양원에서 장기간

간병 치료를 받고 있다.

한국 뇌졸중 역학

· 2010년 한국 통계청 사망통계에 의하면 약 26,500명이 뇌졸중으로 사망(인구 10만 명당 53.2명)했다.

· 20분마다1명이 뇌졸중으로사망했다.

· 뇌졸중으로 인한 사망은 전체 사망의 약 10%였다.

· 암으로 인한 사망은 다음 사망원인 2위를 차지했다.

· 단일 장기질환 사망 원인 1위를 차지했다.

· 2004년 심사평가원 청구자료와 사망 통계로 추정한 2004년 뇌졸중

발생은 연간 약 105,000명이었고 5분마다 뇌졸중이 발생하고 있다.

· 뇌졸중 발생률은 연간 인구 10만명당216건(남자 213건, 여자220건)이며,

· 나이가 증가할수록 급격하게 증가하여,

· 44세 이하에서는 연간10만명당 20건으로 낮지만,

· 85세 이상에서는 연간 10만명 당 3,297건이었다

· 뇌졸중의 유형별로 2009년에는 전체 뇌졸중 중 허혈성 뇌졸중이 76.1%, 출혈성 뇌졸중이 23.9%를 차지하였다.

· 2005년국민건강 조사에 의하면 19세이상의 성인에서 의사에의해 진단된적이있는 뇌졸중 평생 유병률은 인구1,000명당 15.9명(남 자16.44명, 여자15.37명)으로 나타났다.

· 령별로는 406.53명, 5024.26명, 6057.96명이며, 70세 이상에서는 67.45명으로 50 이후 유병률이 급격하게 증가한다.

· 인구 고령화로 인한 뇌졸중 발생 증가와 사망률 감소로 뇌졸중 후유장애를 앓고 있는 사람들이 증가하고 있으며, 이는 향후 막대한 경제적 부담이  것이다.

· 2005년 자료를 바탕으로 추정한 뇌졸중으로 인한 경제적 부담은 약 3조 7천억 원을 상회할 것으로 평가되었다.

· Compared to people who do not smoke, people who smoke are more than twice as likely to suffer an ischemic stroke.
· It is estimated that the total cost of managing the health of American stroke survivors, including stroke treatment costs, medication costs, and income not earned due to inability to work due to stroke, costs $3.4 billion per year.
